Colorectal cancer (CRC) is a major cause of cancer-related morbidity and mortality. Most colorectal cancers derive from benign precursor lesions, so-called adenomatous polyps, over a long period of time. Colorectal cancer screening is based on the detection of precancerous polyps and early stage CRC in asymptomatic individuals to reduce CRC incidence and mortality. The protective effect of screening programs can be improved by increasing the screening rates. Apart from the established examinations, CT colonography (CTC) has been proposed as an optional test for colorectal cancer screening. The detection rates of CTC for large polyps and cancer are similar to the ones of colonoscopy and superior to stool-based tests. CTC is therefore the radiological test of choice for the detection of colorectal neoplasia. It has replaced double contrast barium enema for almost all indications. As a minimally invasive procedure, CTC has a high safety profile and good patient acceptance. The evaluation of extracolonic organs in addition to the colon can increase examination efficacy. The option to choose CTC as a CRC screening test has the potential to increase the overall screening rates. (orig.)

Das kolorektale Karzinom (CRK) ist eine der haeufigsten Ursachen fuer tumorassoziierte Morbiditaet und Mortalitaet. Es entwickelt sich in der ueberwiegenden Mehrheit der Faelle in einem langjaehrigen Prozess aus gutartigen Vorlaeuferlaesionen, den sog. adenomatoesen Polypen. Die Kolonkarzinomvorsorge (Screening) basiert auf der Detektion praekanzeroeser Vorstufen und CRK-Fruehstadien in asymptomatischen Individuen mit dem Ziel der Reduktion von CRK-Inzidenz und -Mortalitaet. Der protektive Effekt von Vorsorgeprogrammen kann durch eine staerkere Teilnahme gesteigert werden. Neben dem etablierten Einsatz von Stuhltests und endoskopischen Verfahren spricht viel fuer den optionalen Einsatz der CT-Kolonographie (CTK) in der Dickdarmkrebsvorsorge. Sie hat eine vergleichbar hohe Sensitivitaet wie die Koloskopie fuer die Detektion fortgeschrittener Adenome und Karzinome und ist den stuhlbasierten Tests deutlich ueberlegen. Die CTK ist die radiologische Untersuchung der Wahl zur Detektion kolorektaler Neoplasien. Sie hat den Kolonkontrasteinlauf bereits in fast allen Indikationen ersetzt. Als minimal-invasive Untersuchung hat sie ein hohes Sicherheitsprofil und gute Patientenakzeptanz. Durch die zusaetzliche Detektion extrakolischer Pathologien kann die Effizienz der Untersuchung gesteigert werden. Der optionale Einsatz der CTK in der Vorsorge kann dazu beitragen, die Teilnahme an Programmen zur Dickdarmkrebsvorsorge zu steigern. (orig.)Additional details
