Ytterbium 169 citrate in the diagnosis of lung opacities of cancerous origin

Description

Lung scintigraphy for tumour exploration has been widely studied for some years, but unfortunately with the many radioisotopes used at present this examination is not entirely reliable. It seemed interesting therefore to investigate a new tracer, 169Yb citrate, the properties of which were demonstrated recently by HISADA. To estimate the specificity of this tracer we chose 62 records of different bronchopulmonary diseases. After an introductory review of various diagnostic methods the physical and physiological characteristics of ytterbium citrate and its method of use are described, then the records examined are presented and our thoughts and conclusions discussed. 169Yb citrate possesses excellent biophysical properties for tumour scintigraphy but this isotope, though causing no radioactive pollution, delivers an appreciable irradiation dose to the patient examined. It has a positive tropism for pathological lung images. The fixation index of the documents taken separately, apart from that of the 14th day, cannot distinguish between benign and malignant diseases. This is possible with the kinetic uptake curve and the index ratios for the 2nd and 14th days. With these diagnostic criteria the overall results are better than those obtained with other commonly used radioisotopes: true positives 70%, false negatives 11%, false positives 4.5%

Abstract (French)

Depuis quelques annees la scintigraphie pulmonaire a visee tumorale a ete largement etudiee. Malheureusement les nombreux radioisotopes utilises ne permettent pas de se fier totalement a cet examen. C'est pourquoi il nous a paru interessant d'etudier un nouveau traceur, le citrate d'Yb169, dont les proprietes ont ete recemment mises en evidence par HISADA. Afin d'evaluer la specificite de ce nouveau traceur, nous avons choisi 62 dossiers de differentes affections bronchopulmonaires. Apres une introduction rappelant les differentes methodes diagnostiques, nous decrivons les caracteristiques physiques et physiologiques du Citrate d'Ytterbium169 ainsi que sa technique d'utilisation, puis nous presentons les dossiers etudies avant d'exposer nos reflexions et nos conclusions. Le Citrate d'Yb169, possede les criteres biophysiques d'un excellent isotope pour la scintigraphie tumorale. Mais s'il ne provoque pas une pollution radioactive, l'Yb169 entraine une dose d'irradiation non negligeable pour le malade soumis a l'examen. Il a un tropisme positif pour les images pulmonaires pathologiques. L'index de fixation des documents pris isolement, excepte celui du 14e jour, ne permet pas de differencier les affections benignes et malignes. Ceci est possible avec la courbe cinetique de fixation et les rapports des index des 2e et 14e jour. Avec ces criteres diagnostiques les resultats d'ensemble sont superieurs a ceux des autres radioisotopes couramment utilises: vrais positifs 70%, faux negatifs 11%, faux positifs 4,5%
