Dislocations and fractures of the ossicles - HR-CT of the injured petrous bone

Description
As in traumatology of the head the HR-CT of the injured petrous bone is now the imaging method of choice. With axial and coronary slices one can show not only the different parts of the ear but also the course of the facial nerve and particulary the ossicles in the tympanon. The fractures are nice to show, also the soft tissue lessions like brainprolaps in the tympanon or hematotympanon because of the better density resolution in comparison to the conventional tomography with rather higher radiation dose especially for the eye lenses. Important are the slice thickness of 1.5 mm with special examination technique, that also small fractures of the pyramid particularly the labyrinthous organ and of the ossicles mostly associated with dislocations are detectable. Regarding that with the clinical possibilities there are particular consequences for the otosurgeon. The ossicular disclocations of other than traumatic cause are rare. (orig.)
Abstract (German)
So wie in der Traumatologie des cavum cranii hat die hochaufloesende Computertomographie bei Pyramidenverletzungen die uebrigen bildgebenden Verfahren abgeloest. Mit axialen and koronaren Schichten sind nicht nur die einzelnen Abschnitte des Ohres darzustellen, sondern auch der Verlauf des N. facialis und insbesonders die Gehoerknoechelchen im Tympanon. Die Frakturen sind mit dieser Technik gut zu erfassen, ebenso wie weichteildichte Veraenderungen in den Hohlraeumen, z.B ein Hirnprolaps oder ein Haematotympanon durch die ungleich bessere Dichteaufloesung im Vergleich zu den konventionellen Schichtverfahren mit ihrer hoeherer Strahlenbelastung, insbesonders fuer die Augenlinsen. Wesentlich sind die 1,5 mm dicken Schichten mit spezieller Einstelltechnik, so dass auch kleine Frakturen der Pyramide, insbesonders des Labyrinthorgans aber auch an den Ossikula, meist mit Dislokationen vergesellschaftet, zu erkennen sind. Daraus ergeben sich fuer den Operateur zusammen mit den klinischen Moeglichkeiten die entsprechenden Konsequenzen. Selten sind die Ossikuladislokationen nicht traumatischer Genese.
