Oral teratoma (epignathus) associated with congenital giant cervical teratoma: case report and review of therapeutic options

Objective: to report the case of an oral teratoma associated with congenital cervical teratoma, highlighting the severity and associated therapeutical difficulties. Methodology: The study was carried out through the evaluation of the medical record of a patient with diagnosis of oral teratoma associated with congenital giant cervical teratoma in July 2017, at the secondary hospital in the interior of the state of Ceará, after the signing of the Free and Informed Consent Form by the mother. Case description: Newborn vaginal delivery, performed at 36 weeks of gestational age. At the late preterm admission, it presented a pediculated bulky lesion protruding from the mouth associated with the large cervical mass on the right with mandibular involvement. Patient progresses with severe respiratory discomfort, airway compression bradycardia and septic shock and pancytopenia. He evolved to death with 16 days of life. Discussion: The most common congenital teratomas are sacrococcygeal, followed by those of gonads, anterior mediastinum, retroperitoneum and oropharynx. Oral teratoma is rarer and comprises less than 2%, which can lead to asphyxia and severe respiratory failure. Conclusion: Oral teratoma can be diagnosed during prenatal care by ultrasound or resonance, and cesarean section is recommended. (author)
