Oral TRH stimulation of the thyroid in patients with thyroid carcinoma

In patients with differentiated thyroid carcinoma high serum TSH-levels enhance 131J-uptake in thyroid remnant and/or metastases. An effective increase of TSH could be achieved by oral administration of thyrotropin releasing hormone (TRH) even after a short T4/T3-withdrawal period so that we recommend a TRH-stimulation in all patients before a diagnostic or therapeutic 131J-application. Adverse reactions to TRH are infrequent and usually shorttimed so that-in contrast to TSH-stimulation - TRH can be given to outpatients without any risk. (orig.)

Abstract (German)

Mit einer oralen TRH-Stimulation laesst sich vor einer diagnostischen oder therapeutischen Jodgabe bei Patienten mit Schilddruesenkarzinomen der TSH-Spiegel im Serum effektiv steigern, wodurch die Bedingungen fuer eine verstaerkte Jodaufnahme in Schilddruesenresten oder stoffwechselaktiven Metastasen verbessert werden. Vergleichbar hohe endogene TSH-Werte sind nur mit einer laengerfristigen Unterbrechung der Hormonsubstitution zu erreichen, die wegen der Gefahr der Metastasenstimulation und aus psychosozialen Gruenden dem Patienten nicht zugemutet werden sollte. Die beobachtete Anzahl von Nebenwirkungen des TRH schraenkt dessen Anwendung nicht ein, da der Allgemeinzustand der Patienten nur kurzzeitig und im allgemeinen in tolerablem Ausmass beeintraechtigt wird. Im Gegensatz zur TSH-Stimulation kann die orale TRH-Stimulation ohne Risiko fuer den Patienten auch ambulant erfolgen. (orig.)
