Sympathetic reflex dystrophy with hypofixation of technetium 99 m pyrophosphates on bone scintigraphy

Description

A patient with sympathetic reflex dystrophy following injury to the right foot presented fairly typical clinical and radiological signs, though the hot phase was particularly short. Bone scan with technetium 99 m pyrophosphates demonstrated hypofixation, which, though extremely rare, has been previously reported in the published literature. The fact that isotopic bone hypofixation can occur during sympathetic reflex dystrophy should be recognised, as far from constituting an argument against the early diagnosis of this affection, it should enable the organic nature of the disorder to be confirmed, and assist the physician in making an early diagnosis

Abstract (French)

Les auteurs rapportent l'observation d'un malade atteint d'une algodystrophie post-traumatique du pied droit dont l'aspect clinique et radiologique etait assez typique malgre une phase chaude particulierement breve, et qui s'est accompagnee d'une hypo-fixation osseuse a la scintigraphie par les pyrophosphates de technetium 99 m. Les auteurs insistent sur le fait que cette hypofixation bien que tres rare, a deja ete signalee dans la litterature. Une telle hypofixation isotopique osseuse doit etre connue au cours de l'algodystrophie, car loin de constituer un argument contre le diagnostic precoce de cette affection, elle doit permettre a la fois d'affirmer l'organicite du trouble, et d'orienter le clinicien vers le diagnotic precoce d'algodystrophie
