Recent concepts concerning cerebral ischaemia in man and their implications in therapeutic evaluations

Description

Recent concepts concerning focal cerebral ischaemia in man and resulting from the data obtained with positron emission tomography are reported. The evolutive steps in the ischaemic process are divided into: perfusion reserve; extraction reserve recruitment; ischaemic penumbra; lesional process and post-lesional developments. Circulatory and metabolic patterns corresponding to these evolutive steps are described and illustrated. The potentials of these data as guide-lines in therapeutic trial methodology are discussed

La synthese des donnees apportees par la tomographie d'emission positonique sur l'evolution de l'ischemie cerebrale focale chez l'homme permet de distinguer les stades de mise en oeuvre de la reserve circulatoire, de recrutement de la reserve d'extraction, de penombre circulatoire, de constitution des lesions, et d'evolution post-lesionnelle. Les troubles du couplage circulation/metabolisme sont susceptibles de correspondre, chez l'homme, a ces differents stades evolutifs
