Managing drug interactions in radiopharmacy. Example of ioflupane [I123]

Description

In nuclear medicine, the existence of drug interactions between the radiopharmaceutical administered and the patient's treatment may result in erroneous or even uninterpretable images, leading to inappropriate patient management. After reviewing the literature on the subject, the case of ioflupane [I123] was chosen to illustrate this point. Ioflupane is a radiopharmaceutical used in brain scintigraphy, whose biodistribution can be altered by certain treatments or psychoactive substances. To complement this example, a national survey was carried out among French radiopharmacies concerning the detection and management of interactions with radiopharmaceuticals in their respective centers. The aim of this work was to ensure that facilities wishing to carry out this activity had the necessary tools at their disposal. The results of the survey show that a majority of radiopharmacists are aware of and interested in detecting drug interactions between the radiopharmaceuticals prescribed and the patient's treatment. However, the lack of recommendations and tools available to them to carry out this activity seems to be a hindrance to its proper implementation. As clinical radiopharmacy is a fast-developing discipline, it seems essential to provide professionals with the tools they need to carry it out properly, with the aim of making the use of radiopharmaceuticals safer, more relevant and more efficient

Abstract (French)

En medecine nucleaire, l'existence d'interactions medicamenteuses entre le medicament radiopharmaceutique (MRP) administre et le traitement du patient peut aboutir a l'obtention d'images erronees voire ininterpretables pouvant mener a une prise en charge inadaptee du patient. Apres la realisation de bibliographie sur le sujet, le cas de l'ioflupane [I123] a ete retenu pour illustrer ces propos. Il s'agit d'un medicament radiopharmaceutique utilise en scintigraphie cerebrale et dont la biodistribution peut etre modifiee suite a la prise de certains traitements ou substances psychoactives. Afin de completer cet exemple, une enquete nationale a ete realisee aupres des radiopharmacies francaises concernant la detection et la gestion des interactions avec les MRP dans leurs centres respectifs. L'objectif de ce travail etait notamment de s'assurer que les etablissements souhaitant realiser cette activite disposaient des outils necessaires. Les resultats de l'enquete montrent qu'une majorite de radiopharmaciens s'averent sensibilises et interesses par la detection des interactions medicamenteuses entre le MRP prescrit et le traitement du patient. Cependant, le manque de recommandations et d'outils a leur disposition pour effectuer cette activite semble etre un frein a sa bonne realisation. La radiopharmacie clinique etant une discipline en plein developpement, il semble indispensable de mettre a disposition des professionnels les outils necessaires a sa bonne realisation dans un but de securisation, de pertinence et d'efficience dans l'utilisation des MRP.
