Analysis of the causes of accidents at the A1 nuclear power plant

Description
The analysis of the two major accidents (1976 and 1977) at the Jaslovské Bohunice A1 nuclear power plant was re-opened. An outline is presented of the analysis of the underlying causes, refined by applying improved root cause analysis based on information currently available. The author supports the conclusion that the two accidents at the A1 NPP occurred primarily because the power plant management failed to introduce an adequate safety management system. (orig.)
