Description
Lumbar spinal stenosis is a condition of polyetiologic origin. It is defined as narrowing of the spinal canal, the nerve root canals or the intervertebral canals. For clinical, diagnostic and therapeutic reasons it may be divided into two main types: central and lateral stenosis with obstruction of the lateral recesses. Plain radiographs may be suggestive in central or generalized narrowing only. Diagnosis is confirmed by additional investigations, i.e. myelography and CT. Lumbar myelography allows definitive diagnosis of central spinal stenosis. However, validity regarding visualization of the lateral recesses is limited and a decision on the nature of the obstruction, bony or soft tissue, cannot be made. In contrast, CT does support information on the bony outline of the lateral recess as related to the facet and soft tissue structures in particular. Treatment of choice should be conservative-supportive if clinical symptoms permit. Surgery is indicated when there is intolerable pain, progressive muscle weakness or sphincter dysfunction. Surgery seeks to attain complete decompression of neural elements, if instability is present or imminent spinal fusion has to be considered. Shape, size and configuration of the spinal canal have to be determined prior to any surgical approach. CT fulfills these requirements most admirably. (orig.)
Abstract (German)
Die lumbale spinale Stenose ist ein polyaetiologisches Krankheitsbild. Aus klinischen, diagnostischen und therapeutischen Gruenden ist die Trennung in eine zentrale Form der Lumbalstenose und eine periphere Form mit Einengung der lateralen Rezessus sinnvoll. Den Uebersichtsaufnahmen der LWS koennen zwar Hinweiszeichen auf eine zentrale oder generalisierte Einengung des Lumbalkanals entnommen werden, diese Roentgensymptome haben aber fuer sich allein genommen keine grosse Aussagekraft. Die Myelographie dagegen gestattet die definitive Diagnose zumindest der zentralen Stenose. Die isolierte Einengung des lateralen Rezessus entgeht dem myelographischen Nachweis jedoch oft. Schwierigkeiten bereitet auch die Trennung zwischen ossaeren, ligamentaeren oder diskogenen raumbeengenden Faktoren. Dadurch ist die Myelographie fuer die Operationsentscheidung und Planung von nur begrenztem Wert. Die CT vermittelt den besten Eindruck von den raeumlichen Verhaeltnissen, sichert die Diagnose des engen lateralen Rezessus und gestattet die Differenzierung ossaerer und nicht-ossaerer Veraenderungen. Darueber hinaus gibt sie zuverlaessig Auskunft ueber Art, Lokalisation und Umfang der Stenose. Fuer die Operationsplanung ist sie deswegen von besonderer Bedeutung. Bei der Behandlung der lumbalen Stenose hat sie konservative Therapie den Vorrang. Die Operationsindikation sollte zurueckhaltend gestellt werden. Ziel jeder Operation ist die Dekompression der neuralen Strukturen. Bei bestehender oder drohender Instabilitaet ist die zusaetzliche Stabilisierung angezeigt. (orig.)Additional details
