Study of a new anti-CD37 monoclonal antibody radiolabelled with Lutetium-177 in a B-cell Non-Hodgkin Lymphoma: therapeutic efficacy and mechanisms of action

B-cell Non-Hodgkin Lymphoma (NHL) treatment relies on the anti-CD20 antibody rituximab and chemotherapy. However, some patients become refractory to this therapy. Here, the efficacy of the novel anti- CD37 antibody-radionuclide conjugate 177Lu-lilotomab (BetalutinR) was investigated in three NHL preclinical models and compared to both rituximab and 177Lu-rituximab. The 177Lu-cetuximab was also used as a non-targeting antibody for investigating the biological effects of 177Lu alone. In vitro, clonogenic survival and proliferation assays showed that rituximab and 177Lu-rituximab were more cytotoxic than lilotomab and 177Lu-lilotomab in the radioresistant Ramos cell line. Conversely, in the radiosensitive follicular lymphoma DOHH2 cell line, 177Lu-rituximab and 177Lu-lilotomab showed similar efficacy. Mantle cell lymphoma (Rec-1 cells) showed intermediate radiosensitivity leading to intermediate response to 177Lu-lilotomab. A radiobiological approach discriminating the cytotoxic effects of unlabelled antibodies and of radiation showed that synergy was observed between the cytotoxic effects of 177Lu and of rituximab involved in 177Lu-rituximab efficacy. Conversely, for 177Lu-lilotomab, synergy between 177Lu and lilotomab was only observed in DOHH2 cells. The high response of DOHH2 cells to 177Lu-lilotomab was confirmed in vivo. Mice bearing subcutaneous DOHH2 or Ramos xenografts mouse models were intravenously injected with either 177Lu-labelled- or unlabelled lilotomab and rituximab. 177Lu-lilotomab and 177Lu-rituximab showed the same therapeutic efficacy on DOHH2 tumours, although unlabelled lilotomab was less efficient than rituximab. Conversely, in Ramos tumour xenografts, the lower efficacy of 177Lu-lilotomab compared with 177Lu-rituximab could only be compensated by increasing 177Lu-lilotomab tumour absorbed dose. At the molecular level, tumour cells radiation-response was shown to be dependent on their ability to undergo apoptosis. This was associated with a reduction of G2/M cell cycle arrest, itself under the control of Wee-1 and Myt-1-mediated phosphorylation of cyclin-dependent kinase-1 (CDK1) at tyrosine 15 and threonine 14. In conclusion, these results indicate that 177Lu-lilotomab is an efficient therapeutic tool for NHL, particularly for tumours showing reduced CDK1 phosphorylation levels, such as in transformed follicular lymphoma (DOHH2 cells), and associated with synergistic cytotoxic effects between 177Lu irradiation and lilotomab. Abstract (French)

Le traitement du lymphome B non hodgkinien (NHL) est generalement base sur la combinaison d'un anticorps monoclonal anti-CD20, le rituximab, avec la chimiotherapie. Cependant, de nombreux patients deviennent refractaires au ciblage du recepteur CD20 et ne repondent plus aux traitements. Dans cette these, l'efficacite d'un nouvel anticorps monoclonal anti-CD37 conjugue au Luthetium-177 (177Lu-lilotomab, BetalutinR) a ete etudie dans trois modeles precliniques de NHL et compare au rituximab et 4 sa forme radiomarquee au luthetium-177 (177Lu-rituximab). Le 177Lu-cetuximab a egalement ete utilise comme anticorps non-specifique pour etudier les effets biologiques du 177Lu seul. In vitro, des tests de survie clonogenique et de proliferation ont montre que le rituximab et le 177Lu-rituximab etaient plus cytotoxiques que le lilotomab et le 177Lu-lilotomab dans la lignee cellulaire radioresistante Ramos (modele du lymphome de Burkitt). Inversement, dans la lignee cellulaire radiosensible DOHH2 (modele de lymphome folliculaire transforme), le 177Lu-lilotomab et le 177Lu-rituximab ont montre la meme cytotoxicite. La lignee cellulaire Rec-1 (modele du lymphome du manteau) presentait une radiosensibilite intermediaire entrainant une reponse intermediaire au 177Lu-lilotomab. Une approche radiobiologique discriminant les effets cytotoxiques de l'anticorps seul et des radiations a montre de la synergie entre les effets cytotoxiques du 177Lu et du rituximab dans efficacite globale du 177Lu-rituximab. Inversement, pour le 177Lu-lilotomab, de la synergie n'etait observee que dans le modele radiosensible DOHH2. L'importante efficacite du 177Lu-lilotomab dans le modele DOHH2 a ete confirmee in vivo. Des souris porteuses de xenogreffes sous-cutanees de cellules Ramos ou DOHH2 ont ete traitees par intraveineuse avec le lilotomab, le rituximab et leur version radiomarquee. Le 177Lu-lilotomab et le 177Lu-rituximab ont montre la meme efficacite therapeutique sur les tumeurs radiosensibles DOHH2, alors que le lilotomab non radiomarque etait moins efficace que le rituximab. Inversement, sur les tumeurs radioresistantes Ramos, la plus faible efficacite du 177Lulilotomab compare au 177Lu-rituximab pouvait seulement etre compensee par une augmentation de dose absorbee a la tumeur par le 177Lu-lilotomab. A I'echelle moleculaire, la reponse des cellules tumorales aux radiations a ete montree comme dependante de leur capacite a induire l'apoptose. Cela a ete associe avec une reduction de l'arret en G2/M du cycle cellulaire lui-meme sous le contrdele des phosphorylations mediees par Wee-1 et Myt-1 de la kinase dependante des cyclines-1 (CDK1) sur la tyrosine 15 et threonine 14. En conclusion, ces resultats indiquent que le 177Lu-lilotomab est un outil therapeutique efficace dans le traitement des NHL et particulierement dans les tumeurs montrant une reduction des niveaux de phosphorylations de CDK1, comme dans le modele de lymphome folliculaire transforme (cellules DOHH2), et associe avec des effets cytotoxiques synergiques entre le 177Lu et le lilotomab.
