Evaluate the positive predictive value of labeled-leukocyte scintigraphy performance in the diagnosis of bone and joint infections

Objective. - Evaluate the positive predictive value of labeled-leukocyte scintigraphy in the diagnosis of bone and joint infections with or without orthopaedic implants. Materials and methods. - Retrospective study of 80 patients suspected of bone and joint infections, with or without orthopaedic implants, who underwent bone scintigraphy, labeled-leukocyte scintigraphy and colloid bone marrow scintigraphy. This preliminary study focuses on nonnegative scintigraphy results performed from 2005 to 2010. The final diagnosis conclusion was based on clinical, biological, bacteriological data, imaging techniques, and follow-up. The patients were classified as true or false-positives. The influence of the different parameters on the exam's diagnostic performances was assessed by a statistical analysis. Results. - Among the 80 scintigraphies performed, 55 were positives and 25 were inconclusive. Among the 55 positive scintigraphies, there were 33 true-positives, 10 false-positives and 2 inconclusive (10 patients were lost sight). Positive predictive value was 76.7%. There was no significant difference between sub-groups: with or without orthopaedic implants (P = 0.385), suspected bone infection location (axial or peripheral skeleton) (P = 0.425), with or without antibiotherapy (P = 0.311), technical way of labelling leukocyte (isolated of granulation or mixed leukocytes) (P = 0.892), with or without colloid marrow imaging (P = 0.529). Conclusion. - Our results indicate that positive predictive value was good and different parameters did not significantly modify diagnostic performance of labeled-leukocyte scintigraphy. (authors)

Objectif. - Evaluer la valeur predictive positive de la scintigraphie aux leucocytes marques dans le diagnostic d'infections osteo-articulaires avec ou sans materiel chirurgical. Patients et methodes. - Etude retrospective portant sur 80 patients suspectes d'infection osteo-articulaire, avec ou sans materiel chirurgical, ayant beneficie d'une scintigraphie osseuse, d'une scintigraphie aux leucocytes marques et d'une scintigraphie aux colloides. Ce travail preliminaire porte sur les scintigraphies non negatives realisees de 2005 a 2010. Le diagnostic final a ete obtenu sur un faisceau d'arguments cliniques, biologiques, bacteriologiques, d'imagerie et le suivi. Les patients ont ete classes en vrais et faux positifs. Une analyse statistique de l'influence de differents parametres sur les performances de l'examen a ete realisee. Resultats. - Sur les 80 examens scintigraphiques, 55 etaient positifs et 25 etaient douteux. Sur les 55 examens positifs, il y avait 33 vrais positifs, 10 faux positifs et 2 cas cliniquement douteux (10 patients ont ete perdus de vue). La valeur predictive positive etait de 76,7 %. Il n'y avait pas de difference significative entre les sous-groupes : avec ou sans materiel chirurgical (p = 0,385), localisation osseuse suspectee d'infection (squelette axial versus squelette peripherique) (p = 0,425), avec ou sans antibiotherapie (p = 0,311), technique de marquage des leucocytes (avec separation des polynucleaires ou non) (p = 0,892), avec ou sans scintigraphie medullaire aux colloides (p = 0,529). Conclusion. - Nos resultats indiquent que la valeur predictive positive etait bonne et qu'aucun des differents parametres n'a modifie les performances de la scintigraphie aux leucocytes marques.
