Distribution, sensitivity, and specificity of CEA, TPA, and CEA x TPa marker product values in patients with breast cancer

Sensitivity and specifity of marker product values CEA x TPA are compared to sensitivity and specificity of single marker determinations of CEA and TPA, respectively. Discrimination capacity of the methods was compared in view of blood donors and treated patients with breast cancer. At a given specifity of 99% in blood donors we found that the CEA x TPA product formation leads to a sensitivity of 96% in patients with active disease, whereas in single marker determinations there was only 52% sensitivity for CEA and 70% for TPA. In treated patients without recurrent tumor growth the product value determination showed 62% sensitivity at 99% specificity, whereas single marker determination led to only 21% for CEA and 27% for TPA, respectively. As a result, the product formation CEA x TPA appears superior to single marker determination and should be clinically used. (orig.)
Abstract (German)
Die Verteilung, Sensitivitaet und Spezifitaet der Ergebnisse kombinierter Markermessungen aufgrund einer Produktbildung aus einzelnen Marker-Konzentrationen wird verglichen mit der Sensitivitaet und Spezifitaet bei Messung einzelner Marker (CEA oder TPA). Untersucht wurde die Diskrimination der Bestimmungen CEA, TPA oder CEA x TPA zwischen Blutspendern als Kontrollgruppe und behandelten Patientinnen bei Mammacarcinom mit Metastasen und/oder Rezidiven. Im gesamten Spezifitaetsbereich zeigt sich die diagnostische Treffsicherheit des Produktwertes den Einzelwerten von Antigenen ueberlegen. Bei der Gruppe von behandelten Patientinnen mit Rezidiv und mit Metastasen ergibt der Produktwert bei 99% Spezifitaet eine Sensitivitaet von 96%, die Einzelbestimmung mit CEA ergibt 52% Sensitivitaet und die mit TPA 70%. Bei der Gruppe von behandelten Patientinnen ohne Rezidiv und ohne Metastasen findet man 62% Sensitivitaet des Produktwertes gegenueber 21% fuer CEA bzw. 27% fuer TPA allein. Der Sensitivitaetszuwachs aufgrund der Produktwertbestimmung spricht fuer die kombinierte Anwendung der Antigene CEA und TPA beim Mammacarcinom. (orig.)Additional details
