Organizational root causes for human factor accidents

Accident prevention techniques and technologies have evolved significantly throughout this century from the earliest establishment of standards and procedures to the safety engineering improvements the fruits of which we enjoy today. Most of the recent prevention efforts focused on humans and defining human factor causes of accidents. This paper builds upon the remarkable successes of the past by looking beyond the human's action in accident causation to the organizational factors that put the human in the position to cause the accident.
