Determination of the concentration of thyroid hormone in dialysate and assessment of the loss of thyroid hormone during hemodialysis

In order to determine the hormone concentration in a dialysate, the commercially available radioimmunossay test of Dow-Lepetit for determination of free thyroid hormones in serum had to be modified. The results raised the suspicion that additional protein-bound hormone had passed the membrane. This could be demonstrated by the addition of Thiomersal. In 64 patients examined 1 and 5 hrs after the beginning of hemodialysis the mean total hormone concentration in the dialysate was 12.05 pg/ml for thyroxine and 5.47 pg/ml for triiodothyronine. The resulting loss of total hormone in hemodialysis patients, calculated for 1 week, amounted to 2.25 μg triiodothyronine and 4.87 μg thyroxine. The comparison with the physiologic renal elimination of thyroid hormone shows that the hormone elimination in hemodialysis patients is negligibly small. The quantitative determination of the protein concentration in the dialysate however, that the loss of protein is without clinical relevance. (orig.)
Abstract (German)
Die Zielsetzung dieser Studie war die Bestimmung des Hormonverlustes waehrend einer Haemodialyse. Diese erforderte die Entwicklung eines geeigneten Untersuchungsverfahrens zur Bestimmung der Hormonkonzentration im Dialysat. Hierzu eignete sich der von Dow-Lepetit kommerziell vertriebene Radioimmunoassay zur Bestimmung der freien Schilddruesenhormone im Serum, der lediglich im ersten Teil modifiziert werden musste. Das Ergebnis liess den Verdacht aufkommen, dass zusaetzlich an Eiweiss gebundenes Hormon die Membran passiert haben musste. Der Beweis gelang durch die Zugabe von Thiomersal. Bei 64 untersuchten Patienten 1 und 5 Std. nach Beginn einer Haemodialyse ergab die gemittelte Gesamthormonkonzentration im Dialysat fuer Thyroxin 12,05 pg/ml, fuer Trijodthyronin 5,47 pg/ml. Der daraus resultierende Gesamthormonverlust bei Haemodialysepatienten, hochgerechnet auf 1 Woche, ergab fuer Trijodthyronin 2,25 μg, fuer Thyroxin 4,87 μg. Der Vergleich mit der physiologischen renalen Schilddruesenhormonausscheidung zeigt, dass die Hormonelimination bei Haemodialysepatienten sehr klein ist und praktisch vernachlaessigt werden kann. Es wurde ferner der Versuch unternommen, eine quantitative Bestimmung der Eiweisskonzentration im Dialysat vorzunehmen. Die gemessene Konzentration von ca. 384 ng/ml ist ausserordentlich niedrig und daher wahrscheinlich nicht sehr praezise. In jedem Falle jedoch ist die eliminierte Eiweissmenge ohne jede klinische Relevanz. (orig.)Additional details
