Modified biological behaviour of sup(99m)Tc-pertechnetate in man as a result of a preceding administration of tin (tin effect)

An increased image of blood-filled spaces (Plexus chorioideus, Sinus transversus) was observed in sup(99m)Tc-pertechnetate scintigraphy when it had been preceded by the administration of tin (e.g. of sup(99m)Tc-Sn-diphosphonate). We have called this behaviour the 'tin-effect'. In vitro studies demonstrated binding of about 80% of the administered activity in the blood with a biological half-life of about 44 hr and an effective half-life of about 5.3 hr. 95% of the blood activity was bound to red cells and 5% to plasma. This resulted in an increased radiation dose to the bone marrow of about 530 mrad/mCi sup(99m)Tc-pertechnetate (following tin). The extent of the tin effect decreased with the length of the interval between tin and sup(99m)Tc-pertechnetate administration. Because of the tin effect, sup(99m)Tc-DTPA or sup(99m)Tc-citrate should be used for brain scintigraphy if this has to be performed within the first 5 or 7 days following a bone scintigraphy with a tin-containing radiopharmaceutical. The 'tin effect' might be taken advantage of when labelling red cells and imaging vascular spaces. (orig.)

Bei 99mTc-Pertechnetat-Szintigrammen wurde eine verstärkte Darstellung der blutreichen Organe (Plexus chorioideus, Sinus transversus) beobachtet, wenn eine Zinn-Applikation (z. B. 99mTc-Sn-Diphosphat) vorangegangen war. Dieses Verhalten wurde von uns "Zinn-Effekt" genannt. In vitro-Untersuchungen ergaben eine Bindung von etwa 80% der verabreichten Radioaktivität im Blut mit einer biologischen Halbwertszeit von etwa 44 Std und einer effektiven Halbwertszeit von etwa 5,3 Std. Die relative Verteilung im Blut war 95% in Erythrozyten und 5% im Plasma. Daraus resultiert eine vermehrte Strahlenbelastung des Knochenmarkes von etwa 530 mrad mCi 99mTc-Pertechnetat (nach Zinn). Der Zinn-Effekt nahm mit der Dauer des Intervalls zwischen Sn- und 99mTc-Pertechnetatapplikation ab. Nach vorangegangener Knochenszintigraphie mit einem zinnhaltigen Radiopharmakon ergibt sich wegen des Zinn-Effektes bis etwa 5 oder 7 Tage danach die Notwendigkeit, eine allenfalls nötige Hirnszintigraphie mit 99mTc-DTPA oder 99mTc-Zitrat durchzuführen. Der "Zinn-Effekt" eignet sich für die in vivo-Markierung von Erythrozyten und die szintigraphische Darstellung von vaskulären Räumen.
