Description
The authors present 143 cases of breast carcinoma T1 and T2, N0, N1, N2 and N3 without clinical signs of bone metastasis. They study the value of bone scintigraphy with technetium 99m labelled diphosphonates. One hundred and twelve patients had normal bone scans; 31 patients showed abnormal fixation sites: in 23 (16%) benign bone lesions explained the increase of the bone radioactivity, in 8 cases, scintigraphy was considered to be consistent with diagnosis of bone metastasis. Metastasis has been proved in only 3 cases within a period of 2 years. The detection rate cannot be better than 5,6 per cent. Considering 50 per cent at least of these patients will eventually develop bone metastasis, the false negative rate will be about 45 per cent. Routine bone scan does not seem to be useful for patients with T1 and T2 breast cancer
Abstract (French)
Les auteurs presentent 143 cas de carcinomes mammaires T1 et T2, N0, N1, N2 et N3, sans signes cliniques de metastase osseuse, pour lesquels ils evaluent l'interet de la scintigraphie osseuse aux diphosphonates marques au technetium 99m. Cent douze cas ne presentaient aucune anomalie; trente et un cas revelaient des foyers de fixation anormaux, dont 23 (16%) ont du etre attribues par confrontation radio-clinique a des affections intercurrentes. Les 8 cas restants faisaient supposer une maladie metastatique qui fut confirmee dans un delai moyen de 2 ans pour 3 cas seulement. Le taux de detection est donc au mieux egal a 5,6 p. 100. Si on admet que 50 p. 100 des malades sont destinees a developper une maladie metastatique, qu'elles ont deja au moment de l'examen scintigraphique, le taux de faux negatifs est de l'ordre de 35 p. 100. La scintigraphie osseuse systematique ne peut donc trouver actuellement une place utile dans le bilan des carcinomes mammaires T1 et T2Additional details
