Radiodine treatment of hyperthyroidism with a simplified dosimetric approach. Clinical results

In this article is evaluated the clinical and effectiveness of a simplified dosimetric approach to the iodine-131 treatment of hyperthyroidism due to Graves' disease or uninodular and multinodular toxic goiter. 189 patients with biochemically confirmed hyperthyroidism and performed thyroid ultrasonography and scintigraphy obtaining the diagnosis of Graves' disease in 43 patients, uninodular toxic goiter in 57 patients and multinodular toxic goiter in 89 patients were enrolled in order to be examined. It was found in 28 patients cold thyroid nodules and performed fine-needle aspiration with negative cytology for thyroid malignancy in all cases. Antithyroid drugs were stopped 5 days till radioiodine administration and, if necessary, restored 15 days after the treatment. Radioiodine uptake test was performed in all patients and therapeutic activity calculated to obtain a minimal activity of 185 MBq in the thyroid 24 hours after administration. The minimal activity was adjusted based on clinical, biochemical and imaging data to obtain a maximal activity of 370 MBq after 24 hours. Biochemical and clinical tests were scheduled at 3 and 12 months posttreatment and thyroxine treatment was started when hypothyroidism occurred. In Graves' disease patients a mean activity of 370 MBq (distribution 259-555 MBq) was administered. Three months after treatment and at least 15 days after methimazole discontinuation 32 of 43 (74%) patients were hypothyroid , 5 of 43 (11%) euthyroid and 6 of 43 (15%) hyperthyroid. Three of the latter were immediately submitted to a new radioiodine administration while 32 hypothyroid patients received thyroxine treatment. One year after the radioiodine treatment no patient had hyperthyroidism; 38 of 43 (89%) were on a replacement treatment while 5 (11%) remained euthyroid. In uni-and multinodular toxic goiter a mean activity of 444 MBq (distribution 259-555 MBq) was administered. Three months posttreatment 134 of 146 (92%) patients were euthyroid and 12 of 146% (8%) patients hyperthyroid. Two patients were immediately submitted to a new radioiodine administration. One year posttreatment 142 of 146 (97%) patients were euthyroid while only 4 of 146 (3%) patients showed TSH levels above the normal range. Only 2 of them required thyroxine treatment. The simplified dosimetric method illustrated in this paper is very effective in clinical practice because it permits to avoid resorting to sophisticated but also imprecise quantitative methods. Hypothyroidism should not be considered as a major collateral effect of radioiodine treatment, particularly in Graves' disease. In fact, the pathogenesis of the disease requires an ablative treatment with both surgery and radioiodine treatment and the control of hyperthyroidism and the prevention of relapse are the major clinical targets. Vice versa, hypothyroidism was very uncommon in uni- and multinodular toxic goiter when the dosimetric approach was applied
Abstract (Italian)
In questo documento viene valutata l'efficacia clinica di un metodo dosimetrico semplificato per la determinazione dell'attivita' terapeutica di iodio-131 da somministrare per il trattamento con intento ablativo di pazienti ipertiroidei affetti da morbo di Graves-Basedow e da gozzo uni o multinodulare tossico. Sono stati arruolati 189 pazienti ipertiroidei sottoposti a ecografia e scintigrafia tiroidea con diagnosi di morbo di Graves-Basedow (43 pazienti), gozzo uninodulare tossico (57 pazienti) e multinodulare tossico (89 pazienti). In 28 pazienti sono stati riscontrati noduli tiroidei freddi di cui e' stata effettuata la citoagoaspirazione con esclusione di caratteri citologici di malignita' in tutti i casi. La terapia medica con farmaci tireostatici e' stata sospesa 5 giorni prima del trattamento radiometabolico e, se necessario, ripresa 15 giorni dopo. Tutti i pazienti sono stati sottoposti a studio della iodocaptazione e l'attivita' terapeutica e' stata calcolata in modo da ottenere l'attivita' intra-tiroidea minima di 185 MBq a 24 ore dalla somministrazione. Tale attivita' minima e' stata quindi modulata in base ai dati clinici, laboratoristici e strumentali fino all'attivita' massima intra-tiroidea di 370 MBq dopo 24 ore. I pazienti sono stati sottoposti a controllo clinico e laboristico 3 e 12 mesi dopo il trattamento, instaurando l'opportuna terapia tireo-sostitutiva nei casi di ipotiroidismo. Nei pazienti affetti da mrobo di Graves-Basedow e' stata somministrata una attivita' media di 370 MBq (distribuzione: 259-555 MBq). Tre mesi dopo il trattamento ed almeno 15 giorni dopo sospensione della terapia tireostatica 32 pazienti su 43 (74%) risultavano ipotiroidei, 5 su 43 (11%) eutiroidei e 6 su 43 (15%) risultavano ipertiroidei. Tre pazienti sono stati immediatamente sottoposti a secondo trattamento con radioiodio mentre 32 pazienti ipotiroidei hanno iniziato la terapia tireo-sostitutiva. Il risultato definitivo dopo 12 mesi e' stato di risoluzione del quadro di ipertiroidismo in tutti i pazienti; 38 pazienti su 43 (89%) affetti da morbo di Graves-Basedow sono stati sottoposti a terapia tireo-sostitutiva mentre 5 (11%) sono rimasti eutiroidei. Nei pazienti affetti da gozzo uni e multinodulare tossico e' stata somministrata una attivita' media pari a 444 MBq (distribuzione 259-555 MBq). Tre mesi dopo il trattamento 134 pazienti su 146 (92%) risultavano eutiroidei e 12 su 146 (8%) ipertiroidei. Due pazienti sono stati sottoposti a seconda amministrazione di radioiodio. Un anno dopo il trattamento 142 pazienti su 146 (97%) risultavano eutiroidei mentre in 4 pazienti su 146 (3%) e' stato osservato aumento del TSH: solo in 2 casi fra questi e' stata necessaria terapia sostitutiva. Il metodo dosimetrico illustrato presenta elevata efficacia clinica e consente di evitare complesse procedure dosimetriche, peraltro imprecise. L'ipotiroidismo dopo trattamento non e' piu' considerato un problema clinico nel caso del morbo di Graves-Basedow, dove la patogenesi della malattia impone la radicale ablazione sia chirurgica che radiometabolica del tessuto tiroideo e il controllo dell'ipertiroidismo e la prevenzione delle recidive rappresentano gli obiettivi clinici fondamentali. Viceversa nel caso di noduli iperfunzionanti il metodo illustrato ha dimostrato incidenza estremamente bassa di ipotiroidismo dopo trattamentoAdditional details
