Renal Late Effects of Radio-Chemotherapy in Pediatric Hodgkins Lymphoma (Hl)

Description

To identify the renal long-term effect of radio-chemotherapy (CTH) in childhood and adolescence Hodgkins disease (HD) patients regularly attending the paediatric oncology clinic of National Cancer Institute (NCI). Forty two long-term survivors (LTS) were studied, together with 26 newly-diagnosed (ND) HD patients and 28 healthy controls. During 3 years period, all patients were subjected to thorough clinical history and examinations. Clinical examination was done with laying stress on blood pressure, visceromegaly and the presence of lymphadenopathy. Lab investigations included CBC, ESR, bone marrow biopsy, and urinalysis. Renal function tests and electrolytes determinations were also done. Radiodiagnostic studies were done whenever indicated. The systolic and diastolic blood pressure-mean difference from the 90th percentile of age and sex-matched group was higher in LTS and ND patients. Systolic Hypertension occurred in 9.5% of LTS patients. ESR and anaemia were significantly higher in ND patients. The changes of blood urea nitrogen (BUN), uric acid (UA), γ-glutamyl transferase (γ-GT), serum Na and K were statistically highly significant among the groups. Finally, this study documented that there are renal late effects of radio- CTH in LTS-HD patients as shown clinically in the form of systolic hypertension, and anaemia with laboratory evidence of radio or CTH impacts on renal function in LTS-HD patients. Recommendations regarding the comprehensive follow-up of therapy for HD patients for renal problems should be done especially in children
