CT classification and functional recovery in putaminal hemorrhage
- 1. Yokohama City Univ. (Japan). Faculty of Medicine
Description
33 CTs of putaminal hemorrhage have been classified into 4 types, each of which includes small as well as large (above 4 - 5 cm in the longest diameter) hemorrhages. The activities of daily living (ADL) is evaluated in all 33 cases 6 months after onset. Type I hemorrhage: All cases of Type I hemorrhage have resulted in ADL 1. Type II hemorrhage: Small hemorrhages of Type II have also resulted in ADL 1. The size of the hemorrhage determines the ADL of the Type II hemorrhage. Type III hemorrhage: Small hemorrhages of Type III have resulted in ADL 1. The depth of the hemorrhage and the encroachment upon the knee of the internal capsule are the most important factors in determining the ADL of the Type III hemorrhage. Type IV hemorrhage: All cases Type IV hemorrhages have resulted in ADL 4 - 5. The Standard Language Test of Aphasia (SLTA) was administered in all 17 cases of left-side hemorrhages one month after onset. Type I hemorrhage: All cases of Type I hemorrhage have fallen into the normal range of standard deviation on the SLTA. Type II hemorrhage: Small hemorrhages have fallen into the normal range of standard deviation on the SLTA. This impairment is most prominent in hearing. Type III hemorrhage: Small hemorrhages have fallen into the normal range of standard deviation on the SLTA. This impairment is most prominent in speech. When the hemorrhage is deep enough to encroach upon the knee of the internal capsule, the ability of speech is almost totally lost on the SLTA in cases of the aged. Reading is usually not so markedly impaired on the SLTA in both Type II and Type III hemorrhages. Type IV hemorrhage: The SLTA was not performed for the Type IV hemorrhage because the modalities of language are almost totally lost in this type of hemorrhage. (J.P.N.)
