An example of theranostic in CHU Grenoble Alpes with the implementation of 177Lu-PSMA-1

Prostate Specific Membrane Antigen (PSMA) is a highly relevant target in nuclear medicine due to its overexpression in prostate cancer. The 68Ga/177Lu-PSMA combination is a theranostic agent for the detection and treatment of tumors overexpressing the PSMA target. Eligibility criterion of 177Lu- PSMA-1 therapy are a progressive disease after chemotherapy and a second-generation hormonotherapy and a high positive PSMA PET/CT up-take. In order to be authorized to carry out this type of treatment, French radiopharmacies must draft and submit an Investigational Medicinal Product Dossier (IMPD) to the national agency for medicines and health products. This requirement ensured pharmaceutical quality for radiopharmaceutical drugs without a marketing authorization. Part of this IMPD is intended for analytical methods validation in order to determine the radiochemical purity (RCP) by High Performance Liquid Chromatography (HPLC) and Thin Layer Chromatography (TLC) methods. The results obtained demonstrated the robustness and reliability of the HPLC and TLC analytical methods for the evaluation of the RCP of 177Lu-PSMA-1. A validation of the aseptic process for the preparation of 177Lu-PSMA1 through a media fill test, a sterility test, a bacterial endotoxins test, a pre-filtration bioburden assessment and a filter integrity test, was performed. Finally, three production batches were performed with an automated synthesis device and we validated all the required quality controls. This work enabled the drafting and validation of the 177Lu-PSMA-1's DME, which was accepted by the French national agency for medicines and health products in June 2021. (author)

L'antigene membranaire specifique de la prostate (PSMA) est une cible tres pertinente en medecine nucleaire en raison de sa surexpression dans le cancer prostatique. L'association 68Ga/177Lu-PSMA est une strategie theranostique pour la detection et le traitement des tumeurs surexprimant le PSMA. Le 177Lu-PSMA-1 est indique pour les patients resistants a la castration avec inefficacite ou intolerance de la chimiotherapie et/ou de l'hormonotherapie de derniere generation, avec une captation positive par TEP/CT. En France, cette radiotherapie interne vectorisee est desormais disponible avec autorisation compassionnelle. Afin de pouvoir realiser ce traitement en routine, les radiopharmaciens doivent rediger et soumettre un Dossier de Medicament Experimental (DME) a l'ANSM. Cette exigence permet de garantir la qualite pharmaceutique des medicaments radiopharmaceutiques sans autorisation de mise sur le marche. Une partie de ce DME est destinee a la validation des methodes analytiques afin de determiner la purete radiochimique (PRC) par les methodes de chromatographie liquide haute performance (CLHP) et de chromatographie sur couche mince (CCM). Les resultats obtenus ont demontre la robustesse et la fiabilite des methodes analytiques par CLHP et CCM pour l'evaluation de cette PRC. Une validation du process aseptique de preparation du 177Lu-PSMA-1 par l'intermediaire d'un test de remplissage aseptique, d'un test de sterilite, d'une recherche d'endotoxines, d'une evaluation de la biocharge pre filtration ainsi que d'un test d'integrite du filtre, a ete effectuee. Enfin trois lots de production ont ete realises avec un automate de synthese et nous avons valide tous les controles qualite exiges. Ce travail a permis la redaction et la validation du DME du 177Lu-PSMA-1 qui a ete accepte par l'ANSM en juin 2021. (auteur)
