Chemoradiotherapy in cervical cancer

Description
Objective: To analyze the results of treatment of chemoradiotherapy in cervical cancer in the Pereira Rossell Hospital. Material and Methods: A total of 134 consecutive patients with stage III b II a, in the period of July 2006 - 2008. The patients who received surgery were excluded. The treatment consisted in pelvic radiotherapy (46.8 Gy) with 14 Gy parametrial reinforcement. Brachytherapy consisted of 4 fractions Media dose rate (MDR) with an average total dose to point A 24 Gy. The biological equivalent dose (BED) was calculated for the tumor (GY10) rectum and bladder (GY3), based on linear quadratic model. The chemotherapy was based on weekly cisplatin, with an average of 4 cycles per patient. The result were analyzed by the Kaplan-Meier overall survival (OS) and disease-free survival (SLE) of the study population. The SLE 24 months into the treatment was 93% for both Stages II and III. No statistical differences were found between the two stadiums. The SG, however, showed significant differences throughout the time analyzed (Log-rank test, p = 0.05; Cox's test, p = 0.02) It is 24 months of the start of 95% for stage II and 82% for stage III. Grade 3-4 complication rate was 2.2% RTOG urinary and digestive 3%. Evolutionary Control: initial local control in 89.6% (120) and persistence 10.4% (14); Loco-regional recurrence: 19.4% (17); Metastasis: 5.2% (7) Conclusions: The diagnosis of neoplasm in advanced stages cervix is still prevalent in the hospital population. The high effectiveness of radio chemotherapy is confirmed in the treatment of cervical cancer .The Most recurrences were pelvic lack of control, which is related to the dose of R T received, they shall be increased in cases of large tumor volume
