The presentation of clinical results of radiolabelling of 99mTc-MIBI complex using ultrasound technique for myocardial perfusion SPECT scanning

Description

Background. - The aim of this study was to introduce a new clinical procedure (ultrasound irradiation technique) as an alternative method for the preparation of sestamibi kits as opposed to the boiling water bath technique. The main proposal of this study was to evaluate the clinical application of ultrasound method, preparation of 99mTc-MIBI complex and biodistribution of the heart in comparison with 99mTc- MIBI complex, which was prepared using the boiling water technique. Materials and methods. - Forty MIBI kits were supplied for twenty patient volunteers. 740-3700 MBq (20-100 mCi) freshly eluted solution of Na+99mTcO4- was added to freeze-dry MIBI Kits (The first day up to 20 kits and the second day another 20 kits with the same amount was used). Moreover, twenty kits were labeled applying the ultrasound radiation method as an alternative approach (The vials were sonicated in thermo noted bath (Elma, P = 50 W, Germany) at 65 ± 0.5 C and for 60 ± 1 seconds), and another twenty kits were provided applying boiling water bath method as a standard approach. Twenty patient volunteers (8 men and 12 women; age range 30-72, median 52.45 years) took part in this research. These volunteers had originally been referred to the hospital for myocardial perfusion imaging. The twenty volunteer patients were divided into two groups (each group contained ten people): group A (consisted of 3 men and 7 women with an age range from 36 to 67 years and a mean age of 51.7 years), and group B (involving 5 men and 5 women with an age range from 30 to 72 years and an average age of 50.3 years). The 99mTc-MIBI complex samples provided by ultrasound method were injected into group A subjects intravenously and group B received 99mTc-MIBI vials provided by boiling water bath approach (intravenous injection). In this research, the myocardial perfusion-SPECT was performed applying a rest-stress protocol which is carried-out between two isolated days for all volunteer patients. Results. - The results obtained in this research applying radio-HPLC and ITLC, suggest that radiolabeling of MIBI vial with 99mTc can be achieved with high efficacy and appropriate acceptance applying the ultrasound method as a novel approach. The imaging research applying SPECT indicated that the 99mTc- MIBI complex samples that were prepared applying an ultrasound irradiation approach indicate very similar biodistribution in the heart, thyroid, lung, liver, gallbladder, kidneys, stomach, GI and bladder of the volunteer patients. Non-habitual aggregation of 99mTc-MIBI samples were not observed in our approach. Conclusions. - The results of this research suggest that the sonication approach can be proposed for preparation of 99mTc-MIBI radio-complex samples. The reaction time to provide radio-complex can be considerably decreased compared to the boiling water bath approach. This main development could decrease the potential risk to the patient to inhibit any delay in emergency situations i.e. acute therapy, especially for myocardial infarction patients in clinical practice. Contexte. - Le but de cette etude etait d'introduire une nouvelle procedure clinique (technique d'irradiation par ultrasons) comme methode alternative pour la preparation de kits sestamibi par opposition a la technique du bain d'eau bouillante. La principale proposition de cette etude etait d'evaluer l'application clinique de la methode par ultrasons, la preparation du complexe 99mTc-MIBI et la biodistribution du coeur en comparaison avec le complexe 99mTc-MIBI, qui a ete prepare en utilisant la technique de l'eau bouillante. Materiaux et methodes. - Quarante kits MIBI ont ete fournis pour vingt patients volontaires. 740-3700 MBq (20-100 mCi) de solution fraichement eluee de Na + 99mTcO4- ont ete ajoutes aux kits MIBI lyophilises (le premier jour jusqu'a 20 kits et le deuxieme jour 20 kits supplementaires avec la meme quantite ont ete utilises). De plus, vingt kits ont ete etiquetes en appliquant la methode de rayonnement ultrasonore comme approche alternative (les flacons ont ete soniques dans un bain thermo note [Elma, p = 50 W, Allemagne) a 65 ± 0,5 et pendant 60 ± 1 secondes] et vingt autres kits ont ete fournis en appliquant la methode du bain d'eau bouillante comme approche standard. Vingt patients volontaires (8 hommes et 12 femmes; tranche d'age 30- 72, mediane 52,45 ans) ont participe a cette recherche. Ces volontaires avaient initialement ete referes a l'hopital pour une imagerie de perfusion myocardique. Les vingt patients volontaires ont ete divises en deux groupes (chaque groupe contenait dix personnes): le groupe A (de 3 hommes et 7 femmes avec une tranche d'age de 36 a 67 ans et un age moyen de 51,7 ans) et le groupe B (5 hommes et 5 femmes avec une tranche d'age de 30 a 72 ans et une moyenne d'age de 50,3 ans). Les echantillons du complexe 99mTc-MIBI fournis par la methode des ultrasons ont ete injectes aux sujets du groupe A par voie intraveineuse et le groupe B a recu des flacons de 99mTc-MIBI fournis par approche au bain-marie bouillant (injection intraveineuse). Dans cette recherche, la perfusion myocardique-SPECT a ete realisee en appliquant un protocole de repos-stress qui est realise entre deux jours isoles pour tous les patients volontaires. Resultats. - Les resultats obtenus dans cette recherche appliquant la radio-HPLC et l'ITLC suggerent que le radiomarquage du flacon de MIBI avec du 99mTc peut etre obtenu avec une efficacite elevee et une acceptation appropriee en appliquant la methode par ultrasons comme nouvelle approche. La recherche en imagerie appliquant la SPECT a indique que les echantillons du complexe 99mTc-MIBI qui ont ete prepares en appliquant une approche d'irradiation par ultrasons indiquent une biodistribution tres similaire dans le coeur, la thyroide, les poumons, le foie, la vesicule biliaire, les reins, l'estomac, l'appareil gastro-intestinal et la vessie des patients volontaires. L'agregation non habituelle des echantillons de 99mTc-MIBI n'a pas ete observee dans notre approche. Conclusion. - Les resultats de cette recherche suggerent que l'approche par sonication peut etre proposee pour la preparation d'echantillons de radio-complexe 99mTc-MIBI. Le temps de reaction pour fournir un radiocomplexe peut etre considerablement reduit par rapport a l'approche du bain d'eau bouillante. Ce developpement principal pourrait reduire le risque potentiel pour le patient d'inhiber tout retard dans les situations d'urgence, c'est-a -dire le traitement aigu, en particulier pour les patients atteints d'infarctus du myocarde dans la pratique clinique.
