Evaluation of X-ray procedures in the diagnosis of eye muscle paralysis

Out of 444 patients with paralysis of the eye muscles 187 were examined by plain X-ray films, 144 of were reviewed in order to address the question, whether the X-ray examination was able to elucidate ethiology and pathogenesis of the paralysis. Of 444 patients we found positive results in only 14 per cent. Among traumatic, tumorous or inflammatory lesions we found positive results in 47 to 84 per cent of the patients, so that there was reasonable influence on the following diagnostic procedure. Plain films were rarely able to establish a definite diagnosis suitable for therapy. By comparison, computed tomography delivers additional information, so that special procedures could be omitted. In spite of that plain films of the skull should be the first step of the diagnostic algorithm, because they give the right direction to the subsequent expensive procedures. (orig.)
Abstract (German)
Bei 444 Patienten mit Augenmuskellaehmungen wurden in 187 Faellen Roentgennativuntersuchungen durchgefuehrt, von denen 144 reinterpretiert werden konnten. Dabei wurde die Frage untersucht, ob die Roentgenaufnahmen einen fuer die Therapie relevanten Beitrag zur Aetiologie und Pathogenese der AML lieferten. Bezogen auf die Gesamtzahl der Patienten fanden sich nur in 14% der Faelle positive Ergebnisse. Bei traumatischer, tumoroeser und entzuendlicher Genese waren von 47% bis 84% der Faelle relevante Befunde zu erheben, so dass das weiter diagnostische Procedere richtungsgebend beeinflusst wurde. Die nativdiagnostischen Verfahren fuehren jedoch zumeist nicht zu einer definitiven, therapiefaehigen Diagnose. Die Computertomographie ist dagegen in der Lage, zusaetzliche Informationen zu liefern, so dass die technisch aufwendigen nativdiagnostischen Spezialuntersuchungen heute weitgehend verdraengt worden sind. Die Schaedeluebersichtsaufnahme sollte jedoch am Anfang des diagnostischen Algorithmus stehen, da sie einen gezielten Einsatz der kostenintensiven weiterfuehrenden Untersuchungsverfahren erlaubt.
