In Europe, malignant growths of the maxiliofacial region account for about 3% of all malignant growths. Many patients are treated by radiotherapy, either before surgery, after surgery, as sole therapy, or in combination with chemotherapy. Depending on the technique applied and on the general and local situation of the patient, reactions of the surrounding normal tissue may be observed. These side effects may occur as early or late reactions of the teeth, mucous membranes, periodontium, salivary glands, and bones. In addition, alcohol, nicotine abuse, or generalized disorders may change the prognosis for the worse and aggravate local complications. If radiotherapy is interrupted for any of these reasons, this will of course interfere with the healing process. In consequence, both the radioconsologist and the dentist must know about the effects of ionizing radiation on the different types of tissue in the maxiliofacial region in order to assure early detection and treatment of complications and information of the patient. Abstract (German)
Die Malignome im Kopf-Hals-Bereich machen in Europa etwa 3% aller boesartigen Tumore aus. Ein Grossteil der von dieser Erkrankung befallenen Patienten wird dabei strahlentherapeutisch behandelt. Die Radiotherapie kann als Vorbestrahlung (vor der Operation), als Nachbestrahlung, als alleinige Radiatio bzw. in Kombination mit Chemotherapie erfolgen. Es kann dabei in Abhaengigkeit von gewaehlten Behandlungsplan und der Allgemein- und Lokalsituation des Patienten zu Reaktionen am umgebenden gesunden Gewebe kommen. Diese therapiebedingten Nebenwirkungen treten entweder als Frueh- oder als Spaetreaktion auf und betreffen vor allem die Zaehne, die Schleimhaeute, das Paradontium, die Speicheldruesen und den Knochen. Zusaetzlich verschlechtern Alkoholismus und Nikotinabusus sowie Allgemeinerkrankungen die Gesamtprognose und verstaerken die lokalen Komplikationen. Muss die Strahlenbehandlung deshalb unterbrochen werden, wird natuerlich der Tumorheilungsprozess behindert. Es ist sowohl fuer den Radio-Onkologen als auch fuer den Zahnarzt von hoechster Wichtigkeit, die Wirkung ionisierender Strahlung auf die verschiedenen Gewebe im Kiefer-Gesichts-Bereich zu kennen, um Komplikationen rechtzeitig zu erkennen und zu behandeln bzw. mit dem Patienten zu besprechen.
