Quantification and monitoring of PET/CT data in multicentre trials: The Swiss SAKK 56/07 trial experience

Aim. - To outline the importance of continuous monitoring of quantitative positron emission tomography (PET) data in multicentre trials to minimize quantitative bias in longitudinal intra-patient PET studies in light of the multicentre SAKK 56/07 experience in quantification and monitoring 18F-FDG PET/CT data. Patients and methods. - We collected 64 uniform phantom 18F-FDG PET acquisitions periodically at the enrolling centres (12 European institutions). A core-laboratory analysed them for standard uptake value (SUV) accuracy (desired 1.00±10%) and acceptable image noise was defined by a coefficient of variation (COV) less than 15%. In total, 151 patients 18F-FDG PET acquisitions (baseline and follow-up) were also collected and analysed to verify longitudinal coherence of main acquisition/reconstruction parameters (DICOM tags verification) and patient preparation, in particular the uptake time (desired uptake time [UT] = 60±10 min). Results. - Uniform phantom PET acquisition satisfied the inclusion criteria in 58/64 (89%) examinations. All PET scanner exhibited comparable SUV quantification, but we found large dispersion in terms of noise, with COV ranging 3-15%. Only 1 phantom PET acquisition was out of range with COV = 21.5%. Patient data exhibited important variation in uptake time with UT = 65±10 min (mean±SD), with only 111/151 (74%) patients' examinations satisfying inclusion criteria while 26% were out of range. Conclusions. - Regular monitoring of PET data in multicentre trials is capital to ensure longitudinal intra-patient PET data consistence and minimize quantitative bias while it helps to spread the culture of quality in participating centre. Recent EARL (EANM Research Ltd) standardization and unification of procedures is a welcome step in this direction. (authors)

But de l'etude. - Mettre en evidence l'importance d'un suivi regulier des donnees quantitative en tomographie par emission de positons (TEP) dans le cadre des etudes multicentriques afin de minimiser le biais quantitatif longitudinal et intra-patient de ces etudes. Dans cet article, nous relatons notre experience acquise dans le suivi des donnees quantitatives en TEP/TDM 18F-FDG lors de l'etude multicentrique SAKK 56/07. Methodes de l'etude. - Nous avons examine, de facon centralisee, 64 acquisitions d'un fantome uniforme realise de facon periodique en TEP 18FFDG dans 12 centres europeens participant a l'etude multicentrique SAKK 56/07. L'analyse a concerne la precision de la mesure des standard uptake values (SUV, valeurs acceptes 1,00±10 %) et la valeur du bruit dans l'image, mesuree par le coefficient de variation (COV, valeurs souhaitables < 15 %). Nous avons egalement analyse 151 acquisitions TEP 18F-FDG (bilan initial et de suivi) afin de verifier la coherence longitudinale des parametres DICOM d'interets et la preparation du patient, en particulier, on a verifie le temps entre administration du 18F-FDG et acquisition TEP (souhaite : 60±10 min). Resultats. - Les criteres d'inclusion pour les valeurs SUV mesurees sur les acquisitions TEP du fantome uniforme satisfaisaient les criteres d'inclusion de l'etude dans 58/64 des cas examines (89 %). Toutes les installations TEP ont montre une quantification SUV comparable, neanmoins nous avons trouve une importante variabilite dans les valeurs du bruit, avec des variations du COV entre 3-15 %. Les criteres d'acceptation n'ont pas ete atteints dans un seul cas (COV = 21,5 %). Les donnees patients ont montrees une variabilite importante du temps ecoule entre l'injection et l'acquisition des images (65±10 min, moyenne±SD) : seulement 111/151 (74 %) des donnees patient satisfaisaient les criteres d'inclusion. Conclusions. - Une surveillance continue des donnees TEP dans le cadre des essais cliniques multicentriques est essentielle afin d'assurer la comparabilite des acquisitions intra-patient effectuees a des moments successifs et ainsi minimiser les biais quantitatifs. Cette surveillance vise aussi a promouvoir une culture de la qualite au sein des centres participants. La standardisation et l'unification des procedures en TEP promue par EARL (EANM Research Ltd) ces dernieres annees sont un pas important dans cette direction. (auteurs)Additional details
