Follow-up controls after percutaneous transhepatic choledochus drainage by means of hepatobiliary sequential scintiscanning

For follow-up controls using hepatobiliary sequential scintiscanning with 99mTc-Iodida have proven to be very satisfactory in patients with high bilirubin levels of up to 40 ng/dl. Other than with former IDA agents, this new one has the advantage of absence of renal elimination, so that there will be no overshadowing in the liver parenchyma scan, and biliary excretion can be clearly distinguished from the intestinal activities. Hepatobiliary sequential scintiscanning with 99mTc-Iodida yields information that allows to distinguish in case of rising bilirubin level between a parenchymatous jaundice and an obstruction and thus gives significant criteria for decision about further diagnostics or treatment. As patients may stay in bed during examination, even long scanning periods of up to 24 hours p.i. are not too cumbersome. (orig./MG)

Fuer die Verlaufskontrolle dieser Patienten, bei denen sich hohe Bilirubinspiegel bis zu 40 ng/dl finden, hat sich die hepatobiliaere Sequenzszintigraphie (HBS) mit 99mTc-Iodida bewaehrt. Anders als fruehere IDA-Produkte zeichnet sich diese Substanz durch eine praktisch fehlende renale Elimination aus. Es kommt somit zu einer ueberlagerungsfreien Darstellung des Leberparenchyms und, im Falle der Gallenexkretion, zu einer klaren Abgrenzung der Darmaktivitaet. Die HBS mit 99mTc-Iodida kann daher bei ansteigendem Bilirubinspiegel zwischen einem Parenchymikterus und einer Obstruktion differenzieren und liefert somit wesentliche Entscheidungskriterien fuer das weitere diagnostische und therapeutische Procedere. Da der Patient bei der Untersuchung in seinem eigenen Bett verbleiben kann, bedeutet selbst ein langwieriger Untersuchungsgang bis zu 24 Stunden p.i. fuer ihn keine wesentliche Belastung.
