Imaging method in the diagnosis of pulmonary thromboembolism
Description
Pulmonary thromboembolism has been a common cause of morbidity and mortality. The same has become the third cause of acute cardiovascular disease after acute myocardial infarction and cerebral vascular accident which has produced thousands of deaths per year. Two large multicenter studies have found that although it was reported a mortality rate of up to 50-58% in patients with hemodynamic compromise, even at hemodynamically stable patients the mortality rate varied from 8-15%. Studies of autopsy have shown that up to 10% of the intrahospital deaths are secondary to pulmonary thromboembolism, despite its high incidence have been difficult to diagnose. Within the diagnostic arsenal that has accounted this entity are found: the clinical assessment, laboratory tests such as D dimer, ventilation-perfusion scintigraphy, venous Doppler U.S. of the lower limbs, pulmonary angiography and computed tomography angiography. Helical computed tomography angiography has offered, in daily clinical practice, the first line study in patients with suspected pulmonary thromboembolism, this happens at the end of the decade of the eighties. The computed tomography angiography has offered many advantages with respect to its competitors including availability, cost-benefit, volumetric image acquisition, and with the reconstruction resulting, identification of alternative diagnoses, the ability of valuation of pelvic veins and inferior limbs at the same time and good interobserver concordance. Computed tomography has revolutionized radiology and medicine, it has been noninvasive diagnostic technique of great power that is in continuous development. This technique by its high spatial and temporal resolution to study virtually any organ and has replaced other techniques previously established in the diagnostic algorithms. The benefits of the technique have been clear but is not without limitations. Computed tomography studies should be performed only when they are clinically justified and its usefulness must be demonstrated in all possible scenarios before generalizing their applications. Pulmonary angiography, was in the past the gold standard for the diagnosis of pulmonary thromboembolism, there is sufficient evidence that angioTC can prove, single mode and noninvasive, with increased rates of sensitivity and specificity of the presence of this pathology and optionally be the line first study to confirm the diagnosis. El tromboembolismo pulmonar ha sido una causa frecuente de morbilidad y mortalidad. El mismo ha llegado a ser la tercer causa de enfermedad cardiovascular aguda despues del infarto agudo al miocardio y al accidente vascular cerebral, lo que ha producido miles de muertes por ano. Dos grandes estudios multicentricos han encontrado que aunque fue reportada una tasa de mortalidad de hasta el 50-58% en pacientes con compromiso hemodinamico, aun en pacientes hemodinamicamente estables, la tasa de mortalidad vario de 8-15%. Estudios de autopsias han demostrado que hasta un 10% de las muertes intrahospitalarias son secundarias a tromboembolismo pulmonar; a pesar de su alta incidencia ha sido dificil de diagnosticar. Dentro del arsenal diagnostico con que ha contado esta entidad son encontrados: la valoracion clinica, examenes de laboratorio como el dimero D, centellografia de ventilacion-perfusion, US doppler venoso de miembros inferiores, angiografia pulmonar y la angiografia por TC. La angiografia por tomografia computarizada helicoidal se ha convertido, en la practica clinica diaria, en el estudio de primera linea en pacientes con sospecha de tromboembolismo pulmonar, esto sucede a finales de la decada de los ochentas. La angiografia por tomografia computarizada ha ofrecido muchas ventajas con respeto a sus competidores incluyendo disponibilidad, costo-beneficio, adquisicion de imagenes volumetricas, con su consecuente reconstruccion, identificacion de diagnosticos alternativos, la capacidad de valoracion de venas pelvicas y de miembros inferiores al mismo tiempo y buena concordancia interobservador. La tomografia computarizada ha revolucionado la radiologia y la medicina, ha sido una tecnica diagnostica no invasiva de gran potencial que se encuentra en continuo desarrollo. Esta tecnica por su gran resolucion espacial y temporal permite estudiar practicamente cualquier organo y ha sustituido a otras tecnicas antiguamente establecidas en los algoritmos diagnosticos. Los beneficios de la tecnica han sido claros pero no se encuentra exenta de limitaciones. Los estudios de tomografia computarizada unicamente se deben realizar cuando esten clinicamente justificados y su utilidad debe quedar demostrada en todos los escenarios posibles antes de generalizar sus aplicaciones. La angiografia pulmonar, fue en el pasado el patron de oro para el diagnostico del tromboembolismo pulmonar, hay suficiente evidencia de que la angioTC es capaz de demostrar, de modo simple y no invasivo, con mayores tasas de sensibilidad y especificidad la presencia de esta patologia y eventualmente ser el estudio de primera linea para confirmar el diagnostico.
