Stereotactic radiotherapy of stage I and IIA non-small cell lung carcinoma: a retrospective study of 101 patients

Description

The aim of this paper was to evaluate the efficiency and tolerance of radiotherapy under stereotactic conditions in inoperable patients treated for non-small cell lung carcinoma (NSCLC). A retrospective study of patients treated for stage I and IIa NSCLC between January 2011 and December 2017 by stereotaxy in 4 x 12 Gy, 5 x 8 Gy and 8 x 7.5 Gy in this center shows that, after a median follow-up of 36 months, 104 lesions were analyzed. Local control at 2 years was 78.8% and overall survival 84.2%. Patients, treated in 4 fractions, seemed to have better local control than those in 8 F (p=0.052). There were 77.2% of radiation pneumonitis including 13% of grade 2 and above. Squamous cell carcinoma (SC) and an SUV max >5.2 were predictive factors for progression. As a conclusion, stereotaxy has a high local control rate and seems well tolerated. Patients with CE require treatment with high fractional doses

But: Evaluer l'efficacite et la tolerance de la radiotherapie en conditions stereotaxiques chez les patients non operables traites pour un carcinome bronchique non a petites cellules (CBNPC). Methode: Etude retrospective des patients traites pour un CBNPC de stade I et IIa entre janvier 2011 a decembre 2017 par stereotaxie en 4 x 12 Gy, 5 x 8 Gy et 8 x 7.5 Gy dans notre centre. Resultats: Apres un suivi median de 36 mois, 104 lesions ont ete analysees. Le controle local a 2 ans etait de 78.8% et la survie globale de 84.2%. Les patients traites en 4 fractions semblaient avoir un meilleur controle local que ceux en 8 F( p=0.052). Il y avait 77.2% de pneumopathie radique dont 13% de grade 2 et plus. Le carcinome epidermoide (CE) et un SUV max >a 5.2 etaient des facteurs predictifs de progression.Conclusion: La stereotaxie presente un taux de controle local eleve et semble bien toleree. Les patients ayant un CE necessitent un traitement par des fortes doses par fractions. (auteur)
