The contribution of computed tomography to the differentiation between inflammatory and neoplastic pulmonary disease

Description

Fourty patients suspected of having a bronchogenic carcinoma but who, in fact, had inflammatory pulmonary lesions were examined by computed tomography. The findings were compared with the CT appearances of 40 patients with bronchogenic carcinomas (20 of these underwent surgery). In 28 patients (70%) suspected of having a bronchogenic carcinoma, the CT findings indicated an inflammatory lesion. As might have been expected, there was no single CT criterion which is found only in inflammatory lesions. Chronic inflammatory processes and inflammatory pseudo-tumours (chronic pneumonias and tuberculosis) cannot be distinguished from malignant tumours by CT (12 out of 40 patients, 30%). (orig.)

Abstract (German)

Den computertomographischen Befunden von 40 Patienten mit entzuendlichen Lungenerkrankungen, bei denen zunaechst der roentgenologische Verdacht auf ein Bronchialkarzinom bestand, wurden die CT-Befunde von 40 Patienten mit Bronchialkarzinomen (von denen 20 operiert wurden) gegenuebergestellt. Bei 28 Patienten (70%) mit Verdacht auf ein Bronchialkarzinom sprach der computertomographische Befund eher fuer ein entzuendliches Geschehen. Erwartungsgemaess konnte aber kein einzelnes computertomographisches Kriterium fuer entzuendliche Prozesse gefunden werden. Indurierte Entzuendungsprozesse und entzuendliche Pseudotumoren (meist chronische Pneumonien und Tuberkulosen) lassen sich auch mittels Computertomographie nicht von malignen Tumoren unterscheiden (12 von 40 Patienten, 30%).
