Effects on the lungs and pleura after chemo-embolization of liver tumors with lipiodol-mitomycin

Description
The authors performed 68 liver embolizations in 51 patients. As selectively as possible. Lipiodol, to block the capillary bed, and a chemotherapeutic agent were injected into the level tumors. A CT performed 24 hours after treatment showed the distribution of the contrast medium which is also an indicator of the distribution of the chemotherapeutic agents. It was found that the contrast medium had accumulated not only in the liver but also in the lungs. Here, four different degrees of accumulation were found, according to the amount of Lipiodol used. Embolization of the liver thus involves potential hazards for the lungs, such as microembolisms, pneumonia, and toxic effects of the chemotherapeutic agents. (orig.)
Abstract (German)
Wir untersuchten 68 Embolisationen mit Lipiodol und einem Zytostatikum, die zur Behandlung von Lebertumoren bei 51 Patienten durchgefuehrt wurden. Dabei fanden wir in der Kontroll-CT neben der intrahepatischen Kontrastmittelverteilung auch Lipiodol im Bereich der Lunge und Pleura. Vier Stufen der KM-Akkumulation konnten dabei unterschieden werden. Diese korrelierten mit der Menge des injizierten Kontrastmittels. Der Nachweis in der Lunge von Lipiodal, das auch einen Indikator fuer die Verteilung des verwendeten Zytostatikums darstellt, zeigt moegliche Risiken der Leberembolisationsbehandlung auf. Dabei koennen diese Risiken durch das Kontrastmittel selbst, wie Mikroembolien und entzuendliche Reaktionen, oder durch toxische Effekte des Zytostatikums bedingt sein. (orig.)Additional details
