Intra-operative radiotherapy of malignant tumors: Past, present and perspectives

Intra-operative radiotherapy consists of electron or photon radiation which is used during the operative procedure. The treatment field is arranged very precisely after critical organs have been previously arranged out of the field. The target volume includes the remaining tumor which could not be removed surgically, and the surrounding tumor bed which is also felt to be high risk for recurrence. It is preferable to have as little tumor remaining as possible before the intra-operative treatment radiation is given. Intra-operative radiotherapy was developed less than a quarter century ago in Japon, and it was later used in U.S.A. The accumulated experience in Japan and U.S.A. is rewieved here. Intra-operative radiotherapy has only recently been introduced to France. The biology, physics and medical and technical problems of intra-operative therapy are discussed

La radiotherapie per-operatoire (RTPO) consiste en l'irradiation par un faisceau d'electrons ou de photons, pendant une intervention chirurgicale, d'une region profondement situee, apres eloignement par des procedes artificiels des organes critiques. Le volume-cible est soit une tumeur inextirpable, soit ce qui est preferable, le lit tumoral apres l'exerese aussi complete que possible de la tumeur. La RTPO a ete developpee depuis un quart de siecle au Japon puis aux USA. L'experience acquise par les radiotherapeutes japonais et americains est detaillee.
