Bone scintigraphy in psoriasis

Since 1973 bone scintigraphy using sup(99m)Tc-phosphate-complexes was carried out in 382 patients with psoriasis. For comparison with the results of nuclear medicine, roentgenologic and clinical findings a group af 121 patients with psoriasis aged between 11 and 74 years was compared to a group of 42 patients aged between 20 and 49 years without roentgenologic and clinical signs of psoriasis arthritis. We found by means of isotope investigation that an essentially greater part of the bones adjacent to the joints was involved than was expected according to X-ray and clinical findings. In addition, in 205 patients with psoriasis whole-body scintigraphy, using sup(99m)Tc-MDP, was carried out since 1977/78. In 17 patients we found an increased accumulation of activity in the region of extraarticular structures of the skull as well as of the skeletal thorax. According to these results we conclude that in addition to the clinically and roentgenologically defined psoriatic arthritis in patients with psoriasis an osteopathy may exist, which can only be demonstrated by skeletal scintigraphy and which is localized in bones adjacent to the joints but can also be demonstrated in the region of extraarticular bones. Bei insgesamt 382 Patienten mit Psoriasis wurden seit 1973 Knochenszintigraphien mit sup(99m)Tc-Phosphat-Verbindungen durchgefuehrt. Zum Vergleich der nuklearmedizinischen Ergebnisse mit dem roentgenologischen und dem klinischen Befund wurde eine Gesamtkruppe von 121 rheumaserologisch negativen Patienten mit Psoriasis im Alter von 11-74 Jahren einer Gruppe von 42 Psoriatikern im Alter von 20-49 Jahren gegenuebergestellt. Dabei konnte ein wesentlich hoeherer Befall der gelenknahen Knochenabschnitte nuklearmedizinisch nachgewiesen werden als dies bisher aufgrund der roentgenologischen und klinischen Befunde vermutet wurde. Zusaetzlich wurden seit 1977/78 bei 205 Patienten mit Psoriasis Ganzkoerperszintigraphien mit sup(99m)Tc-Phosphat-Verbindungen angefertigt. Hierbei konnten bei 17 Patienten vermehrte Aktivitaetsanreicherungen im Bereich der extraartikulaeren Knochenabschnitte der Schaedelkalotte und des knoecherne Thorax nachgewiesen werden. Nach den vorliegenden Ergebnissen muss angenommen werden, dass neben der klinisch und roentgenologisch definierten Psoriasis-Arthritis bei Psoriatikern eine nur mit Hilfe der Knochenszintigraphie nachweisbare Osteopatie existiert, die sich vorwiegend im Bereich der gelenknahen Knochenabschnitte manifestiert, sich jedoch auch im Bereich der extraartikulaeren Skelettabschnitte nachweissen laesst und die in ihrer Ausdehnung nur teilweise den roentgenologischen Kriterien der Symmetrie und des Strahlbefalls entspricht.
