Evaluation of the diagnostic value of the single photon emission computed tomography coupled to tomodensitometry in characterization of solitary spinal lesions in cancer patients

Description
Objective. - The purpose of our study was to investigate the contribution of the single photon emission computed tomography coupled to computed tomography (SPECT-CT) in cancer patients and to evaluate its ability to diagnose the solitary spinal lesions observed on planar bone scintigraphy (PBS). Subjects and methods. - From August 2012 to December 2013, all patients with confirmed malignancy, whose PBS showed a solitary spine 'Hot spot', underwent a SPECT-CT. The study included 54 patients (14 men, 41 women), with a mean age of 53.91 years (rang 28-75 years). The patients with apparent bone lesions, important pains or who had not benefited of a supplement by SPECT-CT were excluded from the study. The lesions were classified either as definitely benign, definitely malignant or indeterminate. Results. - Breast and nasopharyngeal carcinoma represented the approximately 72.21% of all cancers, with predominantly breast cancer. The PBS showed solitary spinal lesions in all our patients with a high prevalence of these lesions in the thoracolumbar spine (49 cases) 90%. The further consideration by SPECT-CT was used to classify lesions in benign 35 cases, in malignant 16 cases and indeterminate in 3 cases. Conclusion. - The acquisition SPECT-CT in patients on PBS with solitary spinal lesion improves the accuracy of diagnosis in patients referred for assessment or monitoring in oncology. It should therefore be performed in clinical routine as soon as possible. (authors)

Introduction. - Le but de notre travail est d'evaluer l'apport de la tomographie d'emission monophotonique couplee a la tomodensi- tometrie (TEMP/TDM) en cancerologie et d'apprecier sa capacite a caracteriser les foyers rachidiens uniques sur la scintigraphie osseuse planaire. Patients et methodes. - Entre aout 2012 et decembre 2013, tous les patients porteurs d'une neoplasie prouvee, dont la scintigraphie osseuse, a mis en evidence un foyer rachidien unique, ont beneficie d'une acquisition TEMP/TDM complementaire. L'etude a porte sur 54 patients (14 hommes, 41 femmes), d'age moyen 53,91 ans (extremes : 28-75 ans). Les patients avec lesions osseuses evidentes, douleurs importantes ou qui n'avaient pas beneficie d'un complement par une TEMP/TDM ont ete exclus de l'etude. Les lesions ont ete classees comme : benignes, malignes et indeterminees. Resultats. - Le sein et le cavum representaient pres de 72,21 % de tous les cancers, avec une predominance du cancer du sein. La scintigraphie osseuse planaire a permis d'objectiver des foyers rachidiens uniques chez tous nos patients avec une forte predominance de ces lesions au niveau du rachis dorsolombaire dans 49 cas (90 %). Le complement d'examen par la TEMP/TDM a permis de classer les lesions en benignes dans 35 cas, en malignes dans 16 cas et indeterminees dans 3 cas. Conclusion. - L'acquisition TEMP/TDM ameliore la precision du diagnostic chez les patients adresses pour bilan d'extension ou suivi en cancerologie et presentant a la scintigraphie osseuse planaire un foyer rachidien isole.
