The role of radiotherapy in the management of small cell lung cancer (SCLC)

In the past decade the management of small cell lung cancer (SCLC) has undergone substantial changes with respect to surgery, chemo- und radiotherapy. Due to combined chemotherapy regimens survival rates increased and the role of radiotherapy has been reevaluated. Despite this, the majority of patients treated with chemotherapy alone failed at the primary site. Several randomized studies have shown that irradiation (35 to 40 Gy) following chemotherapy has increased locoregional control rates. There is no uniform opinion about the treatment volume, fractionation schedule or optimal dose of radiotherapy. Recent reports have demonstrated better local control rates with radiation doses in excess of 45 Gy. Prophylactic cranial irradiation with 20 to 40 Gy is widely used in patients with SCLC and generally accepted in patients with complete remission after chemotherapy. Randomized studies have proven that prophylactic cranial irradiation reduces central nervous system relapses, however, no author has reported so far a significant improvement in survival. (orig.)
Abstract (German)
Im letzten Jahrzehnt hat sich die Behandlung des kleinzelligen Bronchialkarzinoms (SCLC) in bezug auf Operation, Chemo- und Radiotherapie entscheidend geaendert. Durch die Kombinationschemotherapie wurden die Ueberlebensraten verbessert, und die Rolle der Radiotherapie wurde neu bestimmt. Die Mehrzahl der Patienten entwickelt jedoch nach alleiniger Chemotherapie ein Lokalrezidiv. Mehrere randomisierte Studien zeigten, dass durch eine konsolidierende Radiotherapie mit 35 bis 40 Gy nach Chemotherapie die lokalen Kontrollraten verbessert wurden. Es gibt keine einheitliche Meinung hinsichtlich des Behandlungsvolumens, der Fraktionierung und der optimalen Dosis. Mehrere Autoren berichteten ueber bessere lokale Kontrollraten mit Strahlendosen von mehr als 45 Gy. Die prophylaktische Schaedelbestrahlung mit 20 bis 40 Gy wird bei Patienten mit kleinzelligem Bronchialkarzinom haeufig eingesetzt und ist bei kompletter Remission nach Chemotherapie allgemein akzeptiert. In randomisierten Studien wurde nachgewiesen, dass die Frequenz an Hirnmetastasen durch die prophylaktische Schaedelbestrahlung gesenkt werden kann. Kein Autor berichtete jedoch bisher ueber eine signifikante Verbesserung der Ueberlebensraten bei Patienten, die eine prophylaktische Schaedelbestrahlung erhalten hatten. (orig.)Additional details
