Study of the impairment of pulmonary perfusion and the change of RVEF in cor-pulmonale

In 51 cases of Cor-pulmonale, pulmonary perfusion scanning using 99mTc-MAA and RVEF measurement with MGEV using 99mTc-RBC were performed simultaneously and compared with the results of Swan-Ganz catheterization. The relationship between the impairment of pulmonary perfusion and the change of RVEF was studied. The result shows that: 1.There is a relationship between the degree of impairment of pulmonary blood flow and the mean pulmonary arterial blood pressure (mPAP). In normal mPAP group, 55.5%(10/18) with pulmonary perfusion impairment and in elevated mPAP group, 92.3%(12/13). 2.There was also a relationship between the degree of pulmonary perfusion impairment and the clinical state of Cor-pulmonale. Patients in de-compensated stage with III degree of pulmonary perfusion impairment, their rate of hypoximia and respiratory failure was 89.5% (17/19), and patients in compensated stage, the rate was 50.0% (7/14). 3.The degree of impairment of pulmonary perfusion has some effect on RVEF. In the compensated group, RVEF was 40.2±5.06% in 20 cases with III degree of impairment, 46±9.2% in 10 cases with II degree and 48.5% in one case with I degree
