The radioimmunoassay of biologically active compounds in parotid fluid and plasma

The potential value of parotid fluid estimations of clomipramine, d-norgestrel, and cortisol have been evaluated for assessment of clinical status and patient compliance. Excellent agreement (r > 0.9) was observed when parotid fluid samples were assayed with and without chromatographic purification. Clomipramine levels following oral dosage (150mg) rose steadily to a maximum in plasma but showed wide fluctuations in parotid fluid. Following an oral dose of d-norgestrel(0.3mg), parotid fluid levels rose steadily to a maximum but plasma response was biphasic making correlation impossible. The sensitivity and high throughput of the d-norgestrel methodology suggests its use in evaluating patient compliance in large scale fertility control programmes. Changes in circulating cortisol concentrations were rapidly and accurately reflected in parotid fluid in normal volunteers. Parotid fluid cortisol showed a market diurnal rythm, suppression to low levels after dexamethasone, and elevation following Synacthen. Low levels after Synacthen stimulation in a patient with secondary adrenal atrophy and constant high levels in Cushingoid patients indicate that parotid fluid cortisol levels could be used for accurate adrenocortical evaluation. The value in rapid screening procedures is stressed since the assay can be performed directly on only 10μl of parotid fluid. (orig./AJ)

Abstract (German)

Zur Beurteilung der klinischen Verfassung und der Reaktion der Patienten auf die Medikamente wurde der Potentialwert von Untersuchungen der Parotisfluessigkeit auf Clominpramin, d-Norgestrel und Hydrocortison bestimmt. Die Untersuchungsergebnisse der Assays der Parotisfluessigkeit ohne und nach chromatographischer Reinigung zeigten eine sehr gute Uebereinstimmung (r > 0,9). Der Clomipramingehalt im Plasma stieg nach oraler Gabe (150 mg) stetig bis zu einem Maximum, wies jedoch in der Parotisfluessigkeit betraechtliche Schwankungen auf. Nach oraler Gabe von d-Norgestrel (0,3 mg) zeigte sich ein stetiger Anstieg der Werte in der Parotisfluessigkeit bis zum Maximum, die Verteilung im Plasma war jedoch zweiphasig, so dass eine Korrelation unmoeglich war. Wegen der Empfindlichkeit und des hohen Durchsatzes der d-Norgestrel-Methode koennte sie gut zur Beurteilung der Reaktion des Patienten in grossangelegten Programmen zur Geburtenkontrolle eingesetzt werden. Schwankungen bei den Hydrocortisonkonzentrationen konnten bei Versuchen mit gesunden Versuchspersonen an den Werten fuer die Parotisfluessigkeit abgelesen werden. Der Hydrocortisongehalt in der Parotisfluessigkeit wies einen Tagesrhytmus auf, Absinken auf niedrige Werte nach Dexamethason, Anstieg nach Synacthen. Eine genaue Kontrolle der Nebennierenfunktion ueber die Bestimmung der Hydrocortisonkonzentration in der Parotisfluessigkeit moeglich ist. Dieser Assay kann direkt mit nur 10 μl Parotisfluessigkeit durchgefuehrt werden. (orig./AJ)
