Enhancement of radiation response in follicular thyroid cancer cells by histone deacetylase inhibitors

Introduction: Radiotherapy represents the most applied non-surgical strategy for cancer treatment. Approximately, 50% of all cancer patients will receive radiation therapy during their course of treatment. Despite the recent technical improvements, many patients still suffer from locally recurrent disease. Combined treatment with chemical radiosensitizers could further increase the therapeutic ratio of radiotherapy. Objectives: To study the effect of the histone deacetylase inhibitors (HDACI), sodium butyrate (NaB) and valproic acid (VA), in the radiosensitivity of poorly differentiated thyroid cancer cells. Materials and Methods: Human follicular thyroid cancer cells (WRO) were incubated with NaB and VA and irradiated with a 137Cs radiation source (7,7 Gy/min) at different radiation doses. The following parameters were studied at different post-irradiation times: cell survival was assessed by clonogenic assay, cell cycle arrest, cell death, DNA damage through the detection of phosphorylated gamma-H2AX by immunofluorescence and DNA damage repair protein expression by Western Blot. Results: Survival fraction at 2 Gy diminished from 68.4±1.4 (control) to 42.0±2.7 (p<0.0001) (NaB) and from 69.0±0.2 (control) to 56.3±1.6 (p<0,01) (VA). Cell death was significantly increased in irradiated cells with or without both HDACI. We observed that cells died predominantly by necrosis especially at longer time points after irradiation. There was a cell cycle arrest at G2/M phase in irradiated cells 6 and 24 h after irradiation. DNA damage analysis showed an increase in the number of foci/cell in those cells treated with both HDACI at 30 minutes, 4 and 24 h after irradiation (p<0.001). We also observed a significant increase at all the studied times in the foci diameter in all the irradiated cells. Interestingly, there was a higher percentage of big foci (>1μm) in the cells treated with both HDACI at 4 and 24 h after irradiation (p<0.01 y p<0.001, 4 and 24 h, respectively). At 4 h after irradiation, Ku80 expression was downregulated by VA (p<0.001) and Rad51 was dowregulated by NaB as well (p<0.001). Conclusion: In these experimental model, HDACI sodium butyrate and valproic acid increased poorly differentiated thyroid cancer cells radiosensitivity through the inhibition of DNA damage repair capacity and cell death increase. This combined treatment modality could provide a potential therapeutic strategy for those locally recurrent and unresponsive differentiated thyroid cancers. (author)

Introducción: La radioterapia representa la modalidad no quirúrgica más efectiva para el tratamiento del cáncer. Aproximadamente el 50% de los pacientes con cáncer recibirán radioterapia a lo largo del tratamiento. A pesar de los avances tecnológicos en los años recientes, un gran número de pacientes presentan recurrencia de la enfermedad. El índice terapéutico de la radioterapia podría mejorarse con el uso de sustancias químicas que sensibilicen las células malignas a los efectos de la radiación ionizante, los radiosensibilizadores. Objetivos: Estudiar el efecto de los inhibidores de histonas deacetilasas (HDACI), butirato de sodio (NaB) y ácido valproico (AV) en la radiosensibilidad de células de cáncer pobremente diferenciado de tiroides. Procedimientos: Células de la línea de cáncer humano tiroideo folicular (WRO) fueron incubadas con NaB y AV e irradiadas con una fuente de 137Cs (1 Gy/min) a diferentes dosis de radiación. Se evaluó la sobrevida celular mediante el ensayo clonogénico, la muerte celular, la distribución en el ciclo celular y el daño al ADN mediante la técnica de inmunofluorescencia de la histona H2AX fosforilada a distintas horas (h) post irradiación. Se analizó la expresión de proteínas que intervienen en la reparación del ADN. Resultados: La fracción de sobrevida a 2 Gy disminuyó de 68,4±1,4 del control a 42,0±2,7 (p<0,0001) en las tratadas con NaB y de 69,0±0,2 a 56,3±1,6 (p<0,01) en aquellas células tratadas con AV. Se observó mediante las diferentes técnicas un aumento significativo en la muerte celular en las células irradiadas con y sin los HDACI, siendo la necrosis predominante a tiempos largos post irradiación. Se observó un arresto en la fase G2/M del ciclo celular en las células irradiadas respecto a los controles sin irradiar a las 6 y 24 h. El análisis del daño al ADN mostró un aumento en el número de focos/célula en aquellas tratadas con los HDACI a los 30 minutos, 4 y 24 h post irradiación (p<0,001). Respecto al diámetro de los focos de daño, hubo un aumento significativo a todos los tiempos en las células irradiadas con y sin los compuestos. Se observó también un porcentaje mayor de focos grandes en las células tratadas con los HDACI a las 4 y 24 h (p<0,01 y p<0,001, 4 y 24 h, respectivamente). Se observó una disminución de la expresión de Ku80 (p<0,01 con AV) y de Rad51 (p<0,001, con NaB) a las 4 h post irradiación. Conclusiones: Los HDACI, butirato de sodio y ácido valproico, aumentarían la radiosensibilidad de células de cáncer de tiroides pobremente diferenciado disminuyendo la capacidad de reparar el daño al ADN y aumentando la muerte celular post irradiación. La combinación de la radioterapia con estos compuestos podría brindar una potencial estrategia terapéutica para este tipo de cáncer de tiroides recurrente y refractario a los tratamientos convencionales.
