SPECT scintigraphy of the liver with 99mTc-red blood cell for the detection of hepatic cavernous hemangiomas

SPECT scintigraphy with 99mTc-labeled red blood cell (RBC-SPECT) was performed in 22 patients (17 foci of hemangiomas, 8 of hepatocellular carcinomas and 2 of others) for evaluating hepatic cavernous hemangioma (HCH). Positive rate for HCH by RBC-SPECT was dependent on tumor size. In HCH less than 2 cm in size, sensitivity, specificity, and accuracy of RBC-SPECT was 20% (1/5), 100% (3/3), and 50% (4/8), respectively. On the other hand, sensitivity, specificity, and accuracy of RBC-SPECT for HCH larger than 2 cm in size was 100% (12/12), 87% (6/7), and 95% (18/19), respectively. Only one case of hepatocellular carcinoma showed a false positive finding. RBC-SPECT is a very and specific means to detect HCH which is larger than 2 cm in size. (author)
