Pre-irradiation of brain tumours from the radiobiologist's point of view

Description

If it seems likely to improve the operability of a tumour by preoperative radiation therapy, a longterm pre-irradiation with doses of 20-40 Gy, and daily doses of appr. 2 Gy should be carried out. Practically, no intraoperative or postoperative complications are expected. On tumours growing diffusely infiltrating and not shrinking concentrically en bloc the operation after such a pre-operative radiological treatment must be as extensive as it would be without pre-irradiation. If however, the biological characteristics of a tumour give reason to assume that loco-regional or far metastases can be set intraoperatively this risk can (without increasing the operations risk) be reduced by a short-time as well as a long-time pre-irradiation. As long as we do not have a possibility to select risk patients, this advantage of the preoperative radiation therapy will remain improvable for most tumours. The indoubtedly good results, however, such as obtained with rectal carcinoma, show that the concept of the pre-operative radiation therapy is correct in its principles. (orig.)

Abstract (German)

Wenn erwartet werden kann, dass die Operabilitaet eines Tumors durch eine praeoperative Strahlentherapie verbessert werden kann, dann sollte eine Langzeitvorbestrahlung mit Dosen zwischen 20 und 40 Gy und taeglichen Einzeldosen von ca. 2 Gy durchgefuehrt werden. Es sind praktisch keine intraoperativen oder postoperativen Komplikationen zu erwarten. Bei Tumoren, die diffus infiltrierend wachsen und nicht en bloc konzentrisch schrumpfen, muss nach einer solchen praeoperativen Strahlentherapie die Operation gleich umfaenglich sein wie ohne Vorbestrahlung. Wenn dagegen auf Grund des biologischen Verhaltens eines Tumors zu vermuten ist, dass intraoperativ loco-regionale oder Fernmetastasen gesetzt werden koennen, dann kann (ohne Erhoehung des Operationsrisikos) durch eine Kurzzeit- wie durch eine Langzeitvorbestrahlung dieses Risiko fuer den einzelnen Patienten vermindert werden. Solange wir keine Moeglichkeit haben, Risikopatienten gezielt auszusuchen, wird sich dieser Nutzen der praeoperativen Strahlentherapie bei den meisten Tumoren grundsaetzlich nicht nachweisen lassen. Die unbezweifelbar guten Ergebnisse z.B. beim Rectumkarzinom zeigen aber, dass das Konzept der praeoperativen Strahlentherapie prinzipiell stimmt. (orig.)
