Indirect MR-arthography in the fellow up of autologous osteochondral transplantation

Description
Purpose: To evaluate the spectrum of findings in indirect MR-arthrography following autologous osteochondral transplantation. Patients and Methods: 10 patients with autogenous osteochondral homografts underwent indirect MR-arthrography at three, 6 and 12 months postoperatively. The MR protocol at 1.5T comprised unenhanced imagings with PD- and T2-weighted TSE-sequences with and without fat-suppression as well as T1-weighted fat-suppressed SE-sequences before and after iv. contrast administration and after active joint exercise. Image analysis was done by two radiologists in conference and comprised the evaluation of signal intensity (SI) and integrity of the osseous plug and the cartilage surface, as well as the presence of joint effusion or bone marrow edema. Results: At three months, all cases demonstrated a significant bone marrow edema at the recipient and donor site that corresponded to a significant enhancement after iv. contrast administration. The interface between the transplant and the normal bone showed an increased SI at three and 6 months in T2-weighted images as well as in indirect MR-arthrography. The marrow signal normalized in most cases after 6 to 12 months, indicating vitality and healing of the transplanted osteochondral graft. The SI of the interface decreased in the same period, demonstrating the stability of the homograft at the recipient site. The osteochondral plugs were well-seated in 9/10 cases. Indirect MR-arthrography was superior to unenhanced imaging in the assessment of the cartilage surface. Cartilage coverage was complete in every case. The transplanted hyaline cartilage as well as the original cartilage showed a significant increase of the SI in indirect MR-arthrography, that did not change in follow up studies. There were no pathological alterations of signal and thickness alterations of the transplanted cartilage in follow up investigations. Conclusion: Indirect MR-arthrography is a useful diagnostic tool following autologous osteochondral transplantation. Assessment of transplant vitality, osseous fixation and stability is possible. Abstract (German)
Zielsetzung: Evaluation der Befunde nach indirekter MR-Arthrographie bei Patienten mit autologer osteochondraler Transplantation. Patienten und Methoden: 10 konsekutive Patienten mit einer Mosaik-Plastik wurden drei, 6 und 12 Monate nach dem Eingriff mittels indirekter MR-Arthrographie untersucht. Das MR-Protokoll bei 1,5T beinhaltete neben nativen PD- und T2-gewichteten TSE-Sequenzen mit und ohne Fett-Suppression T1-gewichtete, fettsupprimierte SE-Sequenzen vor und unmittelbar nach Kontrastmittelgabe sowie nach Bewegung des Gelenkes. Die Bildanalyse wurde gemeinsam von zwei Radiologen durchgefuehrt und beinhaltete die Auswertung der Signalintensitaet (SI) und Integritaet des knoechernen Zylinders ebenso wie der Knorpeloberflaeche, das Vorhandensein eines Knochenmarkoedems und eines Gelenkergusses. Ergebnisse: Drei Monate nach dem Eingriff zeigte sich in allen Faellen in der T2-TSE-Sequenz ein ausgepraegtes Knochenmarkoedem auf Empfaenger- und Donorseite, korrespondierend zu einem deutlichem Signalintensitaetsanstieg nach Kontrastmittelgabe. Die Grenzzone zwischen dem Transplantat und dem originaerem Knochen zeigte eine erhoehte SI nach drei und 6 Monaten in der T2-TSE-Sequenz sowie nach i.v. Gd-DTPA-Gabe. Eine Transplantatdislokation wurde in keinem Fall beobachtet. Als Zeichen der Vitalitaet des osteochondralen Transplantats normalisierte sich das Marksignal in der T1-SE-Sequenz in den meisten Faellen nach 6 bzw. 12 Monaten. Die indirekte MR-Arthrographie war dem Nativ-Scan ueberlegen bei der Beurteilung der knorpeligen Ueberdeckung des Transplantats. In allen Faellen zeigte sich eine komplette Ueberdeckung des knoechernen Zylinders mit Gelenkknorpel. Transplantierter und originaerer hyaliner Knorpel zeigten einen deutlichen Anstieg der SI in der indirekten MR-Arthrographie. Signalalterationen und Beeintraechtigungen der Knorpeldicke wurden in keinem Fall beobachtet. Schlussfolgerung: Die indirekte MR-Arthrographie eignet sich sehr gut zur Verlaufsbeurteilung osteochondraler Transplantate. Die Beurteilung der Vitalitaet und knoechernen Einbindung der Transplantate sowie die Beurteilung der Stabilitaet sind moeglich. (orig.)Additional details
