Radiologic evaluation of an intraorbital gun-shot bullet

Beside conventional X-ray examination, ultrasound and computer tomographic examination are accepted radiologic techniques to assess the extend of damage of intraorbital injuries. With the exception of a few reports in the literature there is agreement that intraorbital metallic foreign bodies and gun-shot bullets represent a contraindication for magnetic resonance (MR) imaging examination because of artificial imaging side effects and the potential of secondary dislocation of the metallic foreign body due to ferromagnetism. By means of the case reported here it is shown that this general opinion is not universally valid. Depending on the elemental metallic composition of foreign bodies and gun-shot bullets it is demonstrated that MR examination allows high-resolution illustration and in some cases is advantageous compared with other radiologic imaging techniques. Abstract (German)

Zur Beurteilung der Lokalisation und des Ausmasses durch Fremdkoerper verursachter intraorbitaler Verletzungen werden neben den konventionellen Roentgen-Uebersichtsaufnahmen der Orbita die Sonographie und die Computertomographie als Methode der Wahl angewendet [2, 4, 5, 12-16, 19, 20, 22, 23]. Mit Ausnahme weniger Autoren besteht die Ansicht, dass metallische Fremdkoerper und Geschossprojektile wegen der Generierung von Artefakten in der Bildgebung und der Gefahr einer sekundaeren Fremdkoerperdislokation aufgrund eines Ferromagnetismus nicht mit Hilfe der Kernspintomographie nachgewiesen bzw. lokalisiert werden duerfen [2-9, 19-23]. Anhand eines Fallberichtes soll gezeigt werden, dass die Verallgemeinerung dieser Ansicht nur eingeschraenkt Berechtigung hat und dass bei metallischen intrakorporalen Fremdkoerpern je nach Art ihrer elementaren metallischen Zusammensetzung die Kernspintomographie durchaus die Methode der Wahl bei der diagnostischen Bildgebung sein kann.
