Effective root cause analysis, A view from a government facility

Description

Introductions are provided to Westinghouse Idaho Nuclear Company and the Chemical Processing Plant. These become the framework for understanding the particular government contractor's view of effective root cause analysis. Also provided is the position of the customer, the Department of Energy (DOE)+. This is done through recent quotes from DOE leaders. Then the symptoms of need for enhanced root cause analysis are presented, along with the eight attributes of an effective root cause analysis process. Finally, the choice of the process is described
