Evaluating the precision of pretest probability and upfront stress myocardial perfusion imaging in diagnosing coronary artery disease

Upfront utility of Stress myocardial perfusion imaging (MPI) in assessment of CAD is still unpopular. In our study MPI was done as a first line evaluation modality in patients with suspected CAD. Analysis was done to evaluate the association of ischemia detection with respect to the pretest probability and final diagnosis of CAD on CAG. Stress-MPI depicted ischemia mostly in intermediate and high-risk group. Strength of association of ischemia on MPI with pretest risk categories was most pronounced for intermediate group patients. Ischemia detection on MPI were congruent with CAG. However, weak correlation was seen among number of territories involvement on these two modalities. Thus, this study reiterates the importance of upfront MPI, especially for the intermediate risk population
