Description
This paper reviews published data on the incidence of radiation myelopathy in patients treated with megavoltage radiation. Two years after completion of radiation treatment with daily fractions of ∼ 2 Gy development of radiation myelopathy is expected in 1% of the patients at 50 to 55 Gy. An incidence of 5% is estimated to occur at ∼ 55 to 60 Gy. At higher doses the risk of spinal cord damage increases steeply. Interpretation of these data has to consider that the incidence of radiation myelopathy increases with increasing time of follow-up. Even after two years no clearcut plateau is observed. High doses per fraction are poorly tolerated by the spinal cord, the α/β-ratio appears to be ∼ 1 Gy. Accelerated schedules with ≤ 2 Gy per fraction but daily doses greatly exceeding 2 Gy may result in a higher risk of radiation myelopathy than conventionally fractionated treatments. This observation seems, at least in part, to be due to a slow component of repair of sublethal radiation damage in the spinal cord. (orig./MG)
Abstract (German)
In der vorliegenden Uebersicht wurden die aus der Literatur verfuegbaren Daten zur Inzidenz chronischer Strahlenmyelopathien beim Menschen nach Megavoltbestrahlung zusammengestellt. Zwei Jahre nach Strahlentherapie mit taeglichen Fraktionen von ∼ 2 Gy muss bei Gesamtdosen von ∼ 50 bis 55 Gy bei 1% der Patienten mit einer radiogenen Myelopathie gerechnet werden. Die 5%-Inzidenz liegt bei ∼ 55 bis 60 Gy. Oberhalb dieser Dosen kommt es zu einem steilen Anstieg des Risikos. Bei der Interpretation dieser Daten muss beruecksichtigt werden, dass die Haeufigkeit radiogener Myelitiden mit zunehmender Nachbeobachtung ansteigt. Auch nach zwei Jahren ist noch kein Plateau erreicht. Generell werden hohe Dosen pro Fraktion vom menschlichen Rueckenmark ausserordentlich schlecht toleriert, der α/β-Wert scheint in der Groessenordnung von ∼ 1 Gy zu liegen. Bei Anwendung akzelerierter Bestrahlungsschemata mit Einzeldosen ≤ 2 Gy, aber Tagesdosen deutlich ueber 2 Gy kann das Rueckenmark staerker geschaedigt werden als nach konventioneller Fraktionierung. Diese Beobachtung scheint zumindest zum Teil auf eine langsame Komponente der Reparatur subletaler Strahlenschaeden im Rueckenmark zurueckzufuehren zu sein. (orig./MG)Additional details
