Molecular heterogeneity of metastatic prostate cancer in view of 68Ga-PSMA and 18FDG avidity. case report series

Description
Molecular heterogeneity of metastatic prostate cancer in view of 68Ga-PSMA and 18FDG avidity. Case report series Nigora Rasulova, Amir Javaid, Osama Ragab, Saud Alenezi. PSMA is a transmembrane protein expressed in prostate tissue and increases in prostate cancer. 68Ga-PSMA is used for staging, restaging, assessment to therapy, and in patient's selection for 177Lu/255Ac PSMA therapy. 18FDG has is used to estimate the tumor's aggressiveness, prognostication in castrate-resistant metastatic prostate cancer. This study aimed to estimate possible heterogeneity of metastases in prostate cancer patients. In 51 patients with 68Ga-PSMA scans, two patients underwent complimentary NaF and 18FDG PET/CT scans, while one patient had complimentary 18FDG scan only. First one was 73-year-old patient with metastatic disease (Gleason's score = 8, PSA = 100) had 68Ga-PSMA avid foci in both prostate lobes (SUVmax = 8.25 and 16.5) and multiple bones (SUVmax = 44.66). 18FDG scan showed increased uptake in multiple bones (SUVmax = 7.1) with no FDG-avidity in the prostate gland. Second one was 56-year-old patient with castrate resistant prostate cancer (Gleason's score = 7, PSA = 2.21) had 68Ga-PSMA uptake in prostate gland (SUVmax = 15.5), obturator node (SUVmax = 2.6), bony lesions (SUVmax = 9.21), and PSMA nonavid multiple lung lesions. 18FDG scan showed increased uptake in lung lesions (SUV max = 14.5), prostate gland (SUVmax = 3.4), obturator nodes (SUVmax = 8.5), and bones (SUVmax = 9.0). Third one was 63-year-old patient (Gleason's score = 7, PSA = 0.03), had 68GaPSMA diffuse uptake in prostate gland (SUVmax = 2.9), and PSMA nonavid left upper lung nodule. 18FDG scan showed no uptake in prostate gland or left upper lung nodule. Our findings showed molecular heterogeneity of metastatic prostate cancer in the combined 68Ga-PSMA/18FDG PET/CT maybe useful to estimate a prognostication and limitations of 177Lu/255Ac PSMA therapy outcome.
