Evaluation of the place and impact of positron emission tomography coupled with computed tomography in the rheumatological management of rheumatoid arthritis in Eastern Normandy. Seven-year retrospective multicenter analysis

Introduction: Pseudo-polyarthritis rheumatica (PPR) is one of the most frequent inflammatory rheumatic diseases in the aged patient. Its clinical polymorphism in both presentation and course, variable response to oral corticosteroid therapy and lack of specific biological markers have led to PPR being considered more as a syndrome than a disease. Metabolic imaging, mainly represented by positron emission tomography coupled with computed tomography (PET-CT), has gradually gained a prominent place in the diagnostic process, and its use in this indication has recently been framed by recommendations issued by nuclear medicine societies. Our study focused on the role and impact of PET-CT in the rheumatological management of PPR, through a retrospective analysis of the ways in which it has been prescribed by hospital rheumatologists in eastern Normandy over the past 7 years. Material and method: The files of patients for whom a PET-CT scan was prescribed by rheumatologists at 5 hospital centers in Eastern Normandy between January 1, 2014 and December 31, 2020 were analyzed. Then, files concerning a PPR diagnosed or suspected by a rheumatologist were selected. Clinico-biological characteristics, morphological imaging results, therapeutic stage, reason(s) for prescription and consequences of PET-CT during management were evaluated. In addition, selected examinations were reviewed using semi-quantitative visual scores. Results: A total of 670 files were analyzed. Then 133 examinations concerning confirmed or suspected PPR were retained. Of these, the diagnosis of PPR was clinically established before PET-CT was performed for 69 examinations (51.9%). Most patients had clinical and/or biological atypia at diagnosis. A CT scan had been performed in 65% of patients. The main reasons for prescribing PET-CT were to find evidence of underlying neoplasia and/or vasculitis, particularly in situations of therapeutic failure. The diagnosis of PPR was confirmed in 99 patients (74.4%) after PET-CT. Large-vessel vasculitis was identified in eleven patients, and a lesion described as suspicious on more than 20% of scans. A dosage adjustment or even a change of therapeutic step was carried out accordingly, and the PET-CT results led to a diagnostic reassessment for 22 patients. Overall, the PET-CT reading scores favoured the diagnosis of PPR compared with the literature. Discussion/Conclusion: This multicenter, 'real-life' study enabled us to evaluate a population of interest representative of the polymorphous presentation and management of PPR, and to analyze regional professional practices regarding the prescription of PET-CT scans in hospitals, both for diagnostic and therapeutic purposes, in the context of possible PPR or already established diagnosis. In addition, the use of validated reading scores enables systematization of the analysis. This work supports the increasingly frequent use of metabolic imaging in the management of clinical pictures suggestive of PPR, which enables both the search for specific sites and the elimination of differential diagnoses

Introduction: La pseudo-polyarthrite rhizomeelique (PPR) est un des rhumatismes inflammatoires les plus frequents chez le sujet age. Son polymorphisme clinique tant dans sa presentation que dans son evolution, la reponse variable a la corticotherapie orale et l'absence de marqueurs biologiques specifiques ont conduit a considerer la PPR plus comme un syndrome et non plus comme une maladie. L'imagerie metabolique, principalement representee par la tomographie par emission de positons couplee a la tomodensitometrie (TEP-TDM), a progressivement gagne une place preponderante dans la demarche diagnostique, et son utilisation dans cette indication a recemment ete encadree par des recommandations issues des societes de medecine nucleaire. Notre travail a porte sur la place et l'impact de la TEP-TDM dans la prise en charge rhumatologique de la PPR, a travers |'analyse retrospective de ses modalites de prescription par les rhumatologues hospitaliers de Normandie orientale au cours des 7 dernieres annees. Materiel et methode: Les dossiers de patients pour lesquels une TEP-TDM a ete prescrite par les rhumatologues de 5 centres hospitaliers de Normandie orientale entre le 1% janvier 2014 et le 31 decembre 2020 ont ete analyses. Puis les dossiers concernant une PPR diagnostiquee ou suspectee par un rhumatologue ont ete selectionnes. Les caracteristiques clinico-biologiques, les resultats d'imageries morphologiques, le stade therapeutique, le(s) motif(s) de prescription ainsi que les consequences de la TEP-TDM au cours de la prise en charge ont ete evalues. De plus, les examens selectionnes ont ete relus a l'aide de scores visuels semi-quantitatifs. Resultats: Un total de 670 dossiers a ete analyse. Puis 133 examens concernant des PPR averees ou suspectees ont ete retenus. Parmi ceux-ci, le diagnostic de PPR etait etabli cliniquement avant la realisation de la TEP-TDM pour 69 examens (51,9%). La plupart des patients presentaient des atypies cliniques et/ou biologiques au diagnostic. Un scanner avait ete realise chez 65% des patients. Les principaux motifs de prescription de TEP-TDM etaient la recherche d'arguments pour une neoplasie et/ou une vascularite sous-jacente, notamment dans les situations d'echec therapeutique. Le diagnostic de PPR a ete confirme chez 99 patients (74,4%) apres la realisation de la TEP-TDM. Une vascularite des gros vaisseaux a ete mise en evidence chez onze patients et une lesion decrite comme suspecte sur plus de 20% des examens. Une adaptation de la posologie voire un changement de palier therapeutique ont ete realises en consequence, et les resultats de la TEP-TDM ont amene a une reevaluation diagnostique pour 22 patients. Les scores de lecture des TEP-TDM etaient globalement en faveur du diagnostic de PPR comparativement aux donnees de la litterature. Discussion/Conclusion: Cette etude multicentrique en 'vie reelle' a permis d'evaluer une population d'interet representative de la presentation polymorphe et de la prise en charge de la PPR, d'analyser les pratiques professionnelles regionales en matiere de prescription de la TEP-TDM en milieu hospitalier, a la fois dans une demarche diagnostique mais aussi dans un but therapeutique, dans un contexte de possible PPR ou de diagnostic deja etabli. De plus, l'utilisation de scores de lecture valides permet une systematisation de l'analyse. Ce travail soutient le recours a l'imagerie metabolique de plus en plus frequent dans le cadre de la prise en charge de tableaux cliniques evocateurs de PPR, qui permet a la fois la recherche de sites specifiques et l'elimination des diagnostics differentiels.
