Radiological aspects of psoriatic osteoarthropathia

Description
Psoriatic osteoarthropathia is a disease of the joints, spine, and bones. It belongs to the category of ''seronegative spondarthritis'', the prototype of which is ankylosing spondylitis. Contrary to earlier assumptions, morphological changes in the X-ray picture are frequent if the joints (psoriatic arthritis) or axial skeleton (psoriatic spondylitis) are affected. Radiographic signs are listed and scintigraphic findings discussed. The morphological changes in the X-ray picture are characteristic, even without psoriatic lesions of the skin. Sacro-iliac changes in the axial skeleton are frequent. In other parts of the skeleton, typical parasyndesmophytes are slightly less frequent, and changes as in ankylosing spondylitis may occur. The diagnosis is easier with whole-body scintiscanning as this technique is able to detect early changes which are not noticeable clinically or radiologically. (orig.)
Abstract (German)
Der Begriff Osteorthropathia psoriatica soll eine Gelenke, Wirbelsaeule und Knochen betreffende Erkrankung charakterisieren. Man kann diese Erkrankung auf Grund von Gemeinsamkeiten (genetisch Praegung u. a.) zur Gruppe der ''seronegativen Spondarthritiden'' zuordnen, als deren Prototyp die Spondylitis ankylosans zaehlt. Im Gegensatz zu bisherigen Vorstellungen finden sich roentgenmorphologische Veraenderungen beim Gelenksbefall (Arthritis psoriatica) und Befall des Achsenskeletts (Spondylitis psoriatica) ueberaus haeufig. Die Studie befasst sich mit der Auflistung der Roentgensymptome und der Bedeutung szintigraphischer Befunde. Die Roentgenmorphologie an den Gelenken zeichnen sich beim typischen Fall durch proliferative Veraenderungen und ein charakteristisches Befallmuster an Haenden und Fuessen aus, so dass auch ohne psoriatische Hauterscheinungen die Diagnose einer ''Arthritis psoriatica sine psoriase'' gestellt werden kann. Am Achsenskelett finden sich in einem sehr hohen Prozentsatz sacroiliitische Veraenderungen, an der uebrigen Wirbelsaeule nicht ganz so haeufig die typischen Parasyndesmophyten, aber auch Veraenderungen wie bei Spondylitis ankylosans. Diagnostisch noch ergiebiger ist die Ganzkoerperszintigraphie, die Veraenderungen im Fruehstadium, noch bevor sie klinische und radiologische Zeichen aufweisen, erkennen laesst.
