Initial clinical experience of intensity-modulated radiotherapy for patients with high-risk prostate cancer

Between January 2005 and June 2010, 23 patients with high-risk prostate cancer were treated with intensity-modulated radiotherapy (IMRT) plus androgen deprivation therapy (ADT) in our institute. The median serum prostate-specific antigen value was 20.8 ng/ml (range, 3.6-95.0 ng/ml). According to the Gleason grading system, 4, 4 and 15 patients were classified as Gleason score ≤6, 7 and ≥8, respectively. Patients received neoadjuvant ADT for 2 years and were treated to a dose of 70 Gy (7 patients) or 74 Gy (16 patients). Median follow-up period after IMRT was 22 months. During the period, PSA relapse was seen in one patient (4.3%) within 1 year after IMRT, while 22 patients (95.7%) showed no recurrence. There were no patients who had Grade 3 toxicity. In conclusion, IMRT plus 2 years of ADT is a possible treatment for high-risk prostate cancer patients without the fear of high-grade toxicity. We continue to evaluate the efficacy and durability of IMRT for high-risk prostate cancer patients. (author)
