Progress in treatment of head and neck cancer. Pt. 1. Chemotherapy

Cancer of the head and neck is commonly diagnosed in an advanced stage with a poor prognosis. New active agents and combinations have recently been identified. By adding chemotherapy to a multimodality approach with surgery and radiation therapy the outcome may be altered. We reviewed the more recently published literature on induction and adjuvant chemotherapy. No survival advantage has been shown for adjuvant chemotherapy. Organ preservation can be achieved with induction chemotherapy followed by limited surgery and radiation in approximately two thirds of the patients with laryngeal carcinoma. Patients achieving a complete response after induction chemotherapy have a better prognosis. Chemotherpy has consistently shown to reduce the frequency of distant metastases. Chemotherapy is indicated only in recurrent or metastatic disease. Induction chemotherapy is limited to laryngeal carcinoma with organ preservation as intent. Local recurrences and intercurrent morbidity are the main reasons for treatment failures. (orig.)

Abstract (German)

Tumoren im Kopf- und Halsbereich werden meist erst in fortgeschrittenem Stadium erkannt. Nur eine Minderheit von Patienten kann mit chirurgischen Massnahmen und Strahlentherapie allein geheilt werden. Neue Therapieansaetze integrieren Chemotherapie in die multimodale Behandlung mit organerhaltender Chirurgie und Radiotherapie. Neuere Studien mit Induktionschemotherapie und adjuvanter Chemotherapie werden diskutiert. In vier groesseren randomisierten Studien konnte kein Ueberlebensvorteil mit Induktionschemotherapie gezeigt werden. Ebenso vermag adjuvante Chemotherapie die Ueberlebenszeit nicht zu beeinflussen. Patienten mit einer kompletten Remission nach Chemotherapie haben eine guenstigere Prognose. Die Haeufigkeit von Fernmetastasen ist konstant vermindert nach Chemotherapie. Induktionschemotherapie erlaubt eine weniger extensive und organerhaltende Chirurgie in rund zwei Drittel der Patienten mit Larynxkarzinomen bei gleichem Behandlungserfolg. Alle bisherigen Studien konnten keinen schluessigen Vorteil fuer neoadjuvante oder adjuvante Chemotherapie zeigen. Induktionschemotherapie scheint nur bei fortgeschrittenen Larynxkarzinomen mit der Absicht zur Organerhaltung angezeigt zu sein. Lokalrezidive sind der Hauptgrund fuer das Scheitern der Therapie.
